Help Patients Build Strength and Independence at Home
Elara Caring is hiring a compassionate Physical Therapist Assistant to serve home health patients throughout lower Fairfield County.
The territory extends from Norwalk to the New York state line, including Stamford and Greenwich.
As a Home Health PTA, you will provide personalized therapy in patients’ homes under the supervision of a Physical Therapist. You will help patients improve their strength, mobility, balance, safety, and confidence while building meaningful one-on-one relationships.

What You’ll Do
- Provide physical therapy treatments according to the established Plan of Care
- Work under the direction and supervision of a licensed Physical Therapist
- Help patients improve mobility, strength, balance, endurance, and independence
- Monitor patient progress and promptly report changes in condition to the supervising Physical Therapist
- Review the Plan of Care before each visit
- Provide treatment according to physician orders and established clinical standards
- Teach patients and caregivers how to safely complete home exercise programs
- Educate patients on the safe use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices
- Reinforce patient goals and encourage participation in therapy
- Complete accurate and timely clinical documentation
- Collaborate with nurses, therapists, physicians, and other members of the care team
What You’ll Bring
- Graduate of an accredited Physical Therapist Assistant program
- Current and unrestricted Connecticut Physical Therapist Assistant license
- At least six months of experience as a Physical Therapist Assistant preferred
- Strong communication, organization, and documentation skills
- Ability to work independently in patients’ homes
- Willingness to travel throughout Norwalk, Stamford, Greenwich, and the assigned coverage area
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Must be able and willing to travel 50%
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
