Part-Time Clinical Staff Pharmacist – Antimicrobial Stewardship (ASP)

This is a remote part time position has a weekend (once every 4 weekends) and holiday rotation (at least 3 a year). Candidate must be willing to obtain additional licensure (CO, OR, MT, IL, CA).

Position Description:

The Clinical Staff Pharmacist is responsible for providing antimicrobial stewardship and other clinical pharmacy services to optimize medication-related healthcare outcomes. The pharmacist primarily supports Antimicrobial Stewardship Program (ASP) services, including prospective audit and feedback, antimicrobial therapy review, and development of evidence-based recommendations. The pharmacist may also provide additional clinical pharmacy services based on client and organizational needs.

Performance Objectives and Responsibilities: Duties may include, but are not limited to:

● Provide antimicrobial stewardship services through prospective review of antimicrobial therapy and identification of opportunities to optimize antimicrobial selection, dosing, route, duration, and monitoring.

● Evaluate antimicrobial therapy using patient-specific clinical information, microbiology and susceptibility results, laboratory data, renal and hepatic function, medication history, allergies, and other relevant clinical information.

● Identify opportunities for antimicrobial de-escalation, escalation, discontinuation, IV-to-oral conversion, dose optimization, duration optimization, and other evidence-based interventions.

● Assess antimicrobial therapy for appropriateness based on indication, suspected or confirmed source of infection, culture and susceptibility data, patient-specific factors, institutional guidelines, and current evidence-based recommendations.

● Develop and communicate clinically appropriate antimicrobial stewardship recommendations to prescribers, pharmacists, and other members of the healthcare team.

● Collaborate with physicians, pharmacists, nurses, infection prevention professionals, microbiology personnel, and other members of the interdisciplinary healthcare team to support antimicrobial stewardship initiatives.

● Apply current infectious diseases and antimicrobial stewardship guidelines, clinical literature, local resistance patterns, and client-specific protocols when evaluating antimicrobial therapy.

● Monitor antimicrobial therapy for efficacy, safety, drug interactions, adverse effects, therapeutic drug monitoring needs, and appropriate dose adjustment based on organ function and other patient-specific factors.

● Accurately document clinical reviews, interventions, recommendations, provider communications, acceptance of recommendations, and other relevant outcomes in applicable systems.

● Support antimicrobial stewardship quality improvement initiatives, data collection, clinical education, protocol development, and other program activities as assigned.

● Provide education and clinical support to pharmacists, pharmacy technicians, healthcare professionals, and other team members regarding antimicrobial stewardship principles and practices.

● Provide other clinical pharmacy services based on client and organizational needs, which may include medication therapy management (MTM), medication reconciliation, transitions of care, pharmacogenomic (PGx) consultations, clinical medication monitoring, adherence and quality initiatives, and other clinical programs as assigned.

● Maintain established clinical, quality, productivity, and documentation standards.

● Follow applicable clinical protocols, client requirements, policies, procedures, antimicrobial stewardship standards, and federal and state regulations.

● Participate in training, quality improvement activities, and development and implementation of new clinical programs as needed.

● Perform other duties as assigned by the Director of Clinical Services, Manager, and Supervisor.

Qualifications and Requirements:

● Graduate of an accredited college of pharmacy with a Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree.

● Active pharmacist license in good standing, with the ability to obtain and maintain licensure in multiple states (CO, OR, MT, IL, CA) as required.

● Clinical pharmacy experience with demonstrated knowledge of infectious diseases, antimicrobial pharmacotherapy, and antimicrobial stewardship principles.

● Completion of a recognized antimicrobial stewardship certificate or training program, completion of a PGY2 Infectious Diseases Pharmacy Residency, or 5+ years of professional experience providing antimicrobial stewardship and/or infectious diseases pharmacy services required.

● PGY1 pharmacy residency training, board certification, or other advanced clinical pharmacy training preferred.

● Strong knowledge of antimicrobial pharmacotherapy, microbiology, antimicrobial resistance, pharmacokinetics/pharmacodynamics, renal dosing, therapeutic drug monitoring, and evidence-based infectious diseases treatment guidelines.

● Ability to independently evaluate complex patient-specific clinical information and develop evidence-based medication therapy recommendations.

● Strong clinical judgment, critical-thinking, and problem-solving skills.

● Excellent verbal and written communication skills, including the ability to effectively communicate clinical recommendations to physicians, pharmacists, and other healthcare professionals.

● Strong organizational and time-management skills with the ability to prioritize clinical reviews and meet established service and productivity expectations.

● Ability to work independently in a remote environment while collaborating effectively with a multidisciplinary clinical team.

● Ability to learn and work across multiple clinical programs and adapt to evolving client and organizational needs.

● Proficient with computer systems, electronic health records, clinical documentation platforms, pharmacy software, clinical drug information resources, and standard productivity applications.

If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

For this position, we anticipate offering an hourly rate of 55 - 91 USD / hourly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
