Job Description:

Osteoporosis Patient Care Coordinator

The Osteoporosis Patient Care Coordinator (UAP) is responsible for supporting the daily administrative, referral, and patient flow needs of the LOA Osteoporosis Center. This position coordinates referrals, prepares patient charts and records, assists with scheduling and communication, rooms patients, and obtains basic patient measurements such as height and weight.

The Patient Care Coordinator works closely with the provider and clinical team to support an organized, efficient, and positive patient experience. This position does not independently perform clinical procedures, obtain or discuss medical histories, provide clinical assessment or education, administer medications, or provide medical advice. Clinical questions or concerns will be appropriately routed to the provider or designated clinical staff.

Responsibilities

· Prepare and organize patient charts for provider review by ensuring required documentation, laboratory results, imaging reports, and relevant medical records are received and accurately updated prior to patient appointments.

· Coordinate and assist with new patient referral processes for the LOA Osteoporosis Center, including obtaining and reviewing referral documentation for completeness, requesting missing records, communicating with referring offices and patients, and assisting with scheduling appointments.

· Obtain missing medical records, laboratory reports, pathology reports, imaging, and other applicable records prior to patient visits.

· Room and escort patients to the appropriate exam area.

· Obtain and document basic patient measurements, including height and weight in accordance with established practice procedures.

· Prepare patients for their appointment by ensuring the exam room and necessary non-clinical materials are ready for the provider.

· Track patient flow throughout the office and assist with maintaining an efficient patient experience.

· Coordinate patient appointments and relay appropriate non-clinical messages to staff.

· Maintain and update provider schedules to ensure patients are scheduled appropriately and appointments are confirmed.

· Provide patients with scheduling and other non-clinical information regarding appointments and testing.

· Route clinical questions, medical concerns, medication questions, and requests for clinical guidance to the appropriate provider or clinical staff.

· Receive prescription refill requests and route them to the appropriate provider or clinical team for review and action.

· Assist with referral follow-up and communication between patients, referring providers, and the Osteoporosis Center.

· Assist with obtaining pre-exam laboratory or diagnostic results in accordance with established office protocols.

· Verify necessary demographic, insurance, scheduling, and administrative information within the medical record.

· Assist with scanning, uploading, filing, and maintaining patient documentation and records.

· Complete assigned documentation and administrative tasks accurately and timely.

· Maintain appropriate confidentiality of patient information and comply with HIPAA and organizational privacy requirements.

· Complete applicable daily operational and safety checklists per OSHA requirements.

· Perform other administrative and patient coordination responsibilities as assigned to support the operational needs of the LOA Osteoporosis Center.

The Osteoporosis Patient Care Coordinator provides administrative, referral, and patient flow support and is not responsible for independently performing clinical functions.

Any clinical questions, concerns, symptoms, medication-related matters, or requests for medical guidance must be referred to the appropriate provider or designated clinical staff.

Qualifications/Competencies

· High school diploma or equivalent required.

· Previous experience preferred in a medical office, healthcare setting, patient coordination, referral coordination, or similar role preferred.

· Knowledge of healthcare terminology and general medical office processes preferred.

· Strong organizational skills and attention to detail.

· Ability to manage multiple priorities in a fast-paced healthcare environment.

· Strong written and verbal communication skills.

· Ability to communicate professionally and compassionately with patients, families, providers, and referring offices.

· Ability to appropriately recognize and maintain the distinction between administrative responsibilities and clinical responsibilities.

· Experience with electronic medical records and scheduling systems preferred.

· Ability to maintain patient confidentiality and handle sensitive information appropriately.

· CMA certification is preferred but is an optional requirement.

This position will require travel to three (3) LOA office locations, including but not limited to North Charleston, Mount Pleasant, and West Ashley.

This position will report to the LOA Osteoporosis Center.
