RN/RPN – Pediatric Home & Community Care

We are currently looking for compassionate, dependable, and self-driven Registered Nurses (RNs) and Registered Practical Nurses (RPNs) to support pediatric and young adult clients throughout Durham Region, including Pickering, Ajax, Whitby, Oshawa, Courtice, and Bowmanville.
Our nurses provide one-to-one nursing care in home, school, and community settings to clients with varying levels of medical complexity.
Pediatric nursing experience is required, and previous complex-care experience is a strong asset. We are willing to provide additional training to the right candidates who have a strong pediatric or community nursing foundation and are interested in developing their complex-care skills.
Scheduling
- Consistent, ongoing weekly availability is required. Nurses must be able to commit to the same agreed-upon shift(s), on the same day(s), each week.
- Continuity of care is extremely important to our clients and families, and we prioritize nurses who are interested in developing long-term relationships with their clients.
- Opportunities may include daytime, evening, overnight, weekday, and weekend shifts depending on current client needs and nurse availability.
Requirements
What You'll Do
- Provide safe, compassionate, one-to-one nursing care in accordance with each client's individualized care plan and medical orders.
- Complete ongoing nursing assessments and monitor clients for changes in respiratory, neurological, physical, cognitive, and overall clinical status.
- Provide complex nursing interventions within your scope of practice and demonstrated competency, which may include tracheostomy care, suctioning, respiratory support, enteral feeding, seizure management, medication administration, and other client-specific interventions.
- Recognize and respond appropriately to changes in client condition and promptly escalate concerns when required.
- Respond appropriately to urgent or emergency situations in the home, school, or community setting.
- Support clients with activities of daily living, mobility, transfers, personal care, and other needs outlined in their individualized care plans.
- Safely operate client-specific medical and mobility equipment in accordance with training and the client's plan of care.
- Collaborate with clients, families, school personnel, physicians, Ontario Health atHome, and other members of the interdisciplinary care team.
- Maintain accurate, complete, and timely clinical documentation in accordance with CNO standards and The Care Company policies.
- Practice safely and competently within your individual knowledge, skill, judgment, training, and professional scope of practice.
Pediatric & Complex Care Experience
Candidates must have pediatric nursing experience, ideally with hands-on experience in one or more of the following:
- Tracheostomy care and suctioning
- Ventilator support
- G-tube and J-tube feeding and care
- BiPAP or CPAP
- Cough assist
- Oxygen therapy and respiratory monitoring
- Seizure monitoring and management
Don't have experience with every intervention listed above? We are willing to train the right candidates!
We encourage nurses with a strong pediatric or community nursing background to apply, even if they are still developing some of their complex-care competencies. Client-specific orientation and training may be provided based on the needs of the assignment.
Previous experience working in home and community care or supporting Ontario Health atHome (formerly HCCSS) clients is also a strong asset.
Other Requirements
- Current registration and good standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
- Valid nursing certificate/registration.
- Current CPR and First Aid certification.
- Valid WHMIS 2015 certification completed within the past 12 months.
- Negative TB test completed within the past 12 months.
- Valid Criminal Record Check with Vulnerable Sector Screening completed within the past 12 months.
- COVID-19 vaccination documentation, subject to accommodation in accordance with the Ontario Human Rights Code.
- Ability to communicate effectively in English, both verbally and in writing.
- Strong nursing assessment, critical-thinking, communication, and clinical judgment skills.
- Ability to identify and promptly report client care concerns and changes in condition to the appropriate Care Coordinator or healthcare provider.
- Ability to work independently in home, school, and community environments while collaborating with the broader healthcare team.
- A valid driver's licence and reliable access to a vehicle is an asset but not required.
