About Total Life

Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states.

Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider.

The role

This is an in-person contract role. We're looking for nurse practitioners in the Dallas-Fort Worth area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits.

You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team.

Schedule

Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow.

What you'll do

• Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment: what matters most to the resident, medication review and deprescribing, cognition (including the Mini-Cog), mood, sleep, nutrition, mobility, fall risk, daily function and advance care planning
• Evaluate eligible residents for weight management, including GLP-1 therapy where clinically appropriate
• Identify residents who would benefit from follow-up care, such as a fuller cognitive review, ongoing medical follow-up, psychiatry or therapy, and coordinate with their primary care provider
• Document every encounter accurately and completely, to Medicare standards, in our EHR
• Represent Total Life professionally with residents, families and community staff
• Travel to assigned senior living communities in the Dallas-Fort Worth area

Requirements

Requirements

• Active, unrestricted Texas APRN license as a nurse practitioner, with a prescriptive authorization number from the Texas Board of Nursing (a multistate RN compact license does not cover APRN practice)
• Current national NP board certification
• Active DEA registration
• Ability to practice under a Prescriptive Authority Agreement with a delegating physician, as required in Texas, including monthly quality assurance meetings and chart review
• Clinical experience caring for adults 65+
• Experience with Medicare documentation and E/M coding
• Your own malpractice insurance (minimum $1M per occurrence, $3M aggregate) that covers in-person care, not just telehealth, or the ability to obtain it before your first site day
• Comfort presenting to groups and speaking with older adults and their families
• A valid driver's license and reliable transportation
• Ability to complete each community's onboarding requirements, which may include a background check, TB screening and immunizations
• Weekday availability for on-site days in the Dallas-Fort Worth area, with hours confirmed in advance

Nice to have

• Experience with comprehensive geriatric assessment
• Familiarity with cognitive screening and staging tools (Mini-Cog, MoCA, FAST); MoCA certification is a plus
• Experience in senior living, long-term care, SNF, hospice, palliative or house-call settings
• Experience prescribing GLP-1s for weight management
• Telehealth experience
• Licenses in more than one state
• Adult-Gerontology NP certification (AGNP/AGPCNP), or Family NP with a geriatric focus or training
