• Must have 2 years of MA experience within the last 5 years

• Active and unencumbered CCMA, CMA, NCMA or RMA certification
• Active BLS Certification (AHA only)
• Annual Physical within last 12 months
• Negative TB within last 12 months
• Hep B Series or Titer
• MMR Series or Titer
• Tdap Vaccine within last 10 years or Titer
• Varicella Series or Titer
