The Opportunity 

As One Medical for Seniors High Risk Programs continues to expand, we are seeking a High Risk Care Operations Coordinator to own and optimize care coordination operations across our High Risk Care programs. Reporting to the Manager, High Risk Field Operations, this role is responsible for managing provider schedules and inventory in 1Life, coordinating patient outreach across multiple markets, and partnering with providers, health navigators, and the broader care team to ensure patients receive timely, high-quality care. 

This is not a task-execution role. The ideal candidate will take ownership of the scheduling infrastructure that underpins our High Risk Care delivery model — not just maintaining it, but actively identifying inefficiencies and recommending improvements. You will be the person who sees the whole picture: patient volume, provider capacity, program needs, and team workflows — and you will use that visibility to ensure we operate at our best.

What you'll likely work on: 

Scheduling and Provider Operations 

• Own and manage provider schedules and inventory in 1Life across multiple High Risk Care programs and markets (including but not limited to Tele-Assist, Field Care, and supporting specialty programs as needed) 

• Facilitate the scheduling of Field Care Coordinators, virtual Nurse Practitioners, and other clinical team members across markets (AZ, WA, TX, GA, CO, and expanding) 

• Maintain accurate, up-to-date provider capacity and availability information to support program operations and leadership decision-making 

• Identify and escalate scheduling conflicts, coverage gaps, and capacity constraints proactively 

• Build and maintain scheduling templates, standard processes, and documentation 

Patient and Care Team Coordination 

• Conduct outbound calls to patients to schedule visits, confirm appointments, and support care coordination needs 

• Field inbound calls and messages from patients and their families; address needs directly or facilitate warm hand-offs to the appropriate care team member 

• Coordinate with providers, health navigators, and the broader care team to ensure seamless transitions of care and appropriate follow-up 

• Support care coordination for patients discharged from ER visits, acute stays, and post-acute facilities — including scheduling follow-up appointments and communicating with patients to prevent readmission 

• Act as a liaison between internal care teams and external medical providers, hospitals, post-acute facilities, and home health organizations where appropriate 

• Monitor patient outreach queues and ensure timely completion of coordination activities 

Strategic Operations and Process Improvement 

• Analyze scheduling patterns, utilization data, and workflow bottlenecks to identify opportunities for optimization 

• Recommend and implement process improvements that increase scheduling efficiency, reduce patient wait times, and improve provider utilization 

• Collaborate with leadership to develop scalable scheduling models that support program expansion into new markets 

• Contribute to the design of standard operating procedures, workflow documentation, and training materials 

• Partner with cross-functional teams (clinical operations, technology, program leadership) to inform systems development and tool improvements 

• Support additional High Risk Care operational initiatives as the program evolves

What you'll need: 

• At least 1 year in a patient-facing role at One Medical, working with high-risk patient populations, as of September 2026

• Demonstrated experience managing complex scheduling across multiple providers, programs, or locations 

• A proven track record of identifying process inefficiencies and implementing improvements 

• Strong written and verbal communication skills; comfort communicating with patients, clinical teams, and leadership alike 

• Ability to work independently, manage competing priorities, and make sound decisions in ambiguous environments 

• Proficiency with scheduling platforms, electronic health records, and general technology tools (Mac iOS, Google Suite); experience with 1Life preferred 

• A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges 

Preferred 

• Experience with workforce scheduling optimization, capacity planning, or dispatch operations 

• Experience in a hospital case management, care coordination, home health, or transition navigator role 

• Familiarity with High Risk or Senior Health patient populations 

• Spanish speaking 

• Certified Medical Assistant (MA) or ability to obtain certification within 6 months of hire 

This is a full-time remote position (40 hours/week) based anywhere in the contiguous United States, with the requirement to work Pacific Time Zone (West Coast) hours, Monday through Friday, 8:00am – 5:00pm. This person will also work with patients and care teams across multiple time zones and markets. This role is remote with limited travel required.

The base compensation range for this role is $22.50 to $33.00 per hour. However, actual compensation packages are based on several factors that are unique to each candidate. These factors include, but are not limited to, job related knowledge and skill set, depth of experience, certifications and/or degrees, and specific work location. 

The total compensation package for certain roles may also include additional components such as a sign-on bonus, annual performance bonus, equity grants in the form of RSUs, medical and other benefits and/or other applicable incentive compensation plans.
