Grosvenor Hart Homes is looking for a Hart Practitioner to join our multidisciplinary team in Kirkby, Knowsley. This is a full-time, customer-facing role working predominantly with care-experienced young people as they prepare for, move into and sustain their own homes. You will also provide some support to children and families.

We provide high-quality, affordable homes alongside tailored employment, wellbeing and practical support. Our aim is simple: remove barriers, build confidence and help people achieve lasting outcomes. Find out more and watch our short film: Social enterprise (grosvenor.com).

You will hold a professional qualification in social work, youth work or a related field. You will bring strong experience of supporting care-experienced young people, children or families, with sound safeguarding knowledge and the confidence to manage complex cases.

This is a chance to do focused, meaningful work with a small caseload. You will build trusted relationships, agree clear plans and keep young people motivated as they work towards independence. You will also help deliver community activities, including life-skills workshops, social events and wellbeing sessions.

What we offer
A competitive salary, generous non-contributory pension, life assurance, 25 days’ holiday plus Bank Holidays, private medical cover, company sick pay and access to a wide range of employee discounts and benefits.

What you will do

  • Build trusted relationships with care-experienced young people before, during and after they move into a Grosvenor Hart Homes property.
  • Assess each young person’s strengths and support needs with them and relevant services.
  • Create clear, personalised plans with measurable goals.
  • Coordinate smooth move-ins and help young people settle into their homes and communities.
  • Keep plans moving: solve problems, remove barriers and support young people to stay focused on their goals.
  • Provide practical and emotional support, including during a crisis.
  • Undertake some direct work with children and families.
  • Help tenants shape and deliver community activities, from cooking and creative sessions to wellbeing and life-skills projects.

What you will bring

  • A professional qualification in social work, youth work or a related field.
  • Strong experience in social care or a similar setting, ideally supporting care-experienced young people.
  • Experience of working with children and families.
  • A sound understanding of safeguarding procedures.
  • Experience of complex case management, assessment and outcome-focused planning.
  • An understanding of social housing and the challenges young people may face when moving towards independence.
  • The ability to build strong relationships, work across agencies and turn plans into action.

This role is based at our Kirkby location, Knowsley. You will work from our hub, in tenants’ homes and at external venues. Occasional travel to Chester and Ellesmere Port will be required. For an informal conversation, contact [email protected] or [email protected].
