OASIS Auditor / Home Health Quality Review Specialist
Deep OASIS Expertise Required | HCS-O or COS-C Preferred (Certification Required Within 90 Days of Hire)
Full Time | Monday–Friday, 8–5 | Some on-call rotation

This is a specialized OASIS compliance role, not a general QA or utilization review position. We need someone who can independently score and defend OASIS-E/E1 assessments — GG and M-item coding, sequencing, and how accuracy drives PDGM case-mix and HHVBP outcomes. Candidates who already hold an active HCS-O or COS-C are strongly preferred. If you don't currently hold one but have deep, hands-on OASIS knowledge, you may still be considered — but certification will be required within 90 days of hire, and you should be prepared to demonstrate that expertise during the interview process. General home health clinical experience without strong OASIS-specific knowledge is not a fit for this role.

What You'll Actually Be Doing

• Auditing OASIS assessments (SOC, ROC, Recert, Follow-up, Transfer, DC) for accuracy, internal consistency, and CMS compliance — not just checking boxes, but catching coding errors that affect case-mix and quality scores
• Reviewing clinical documentation to ensure it supports OASIS answers and ICD-10 coding
• Tracking KPIs tied to OASIS accuracy: outcome measures, case-mix weight impact, HHVBP scores, readmission rates
• Delivering direct, specific feedback to clinicians on OASIS documentation errors — you need to be able to explain why an answer is wrong, not just flag it
• Identifying systemic documentation trends and partnering with leadership on corrective action plans
• Monitoring for patient safety issues and ensuring timely reporting

Required Qualifications

• Demonstrated, deep working knowledge of OASIS-E/E1 scoring and CMS guidance — this will be assessed during the interview process
• HCS-O or COS-C certification strongly preferred; if not currently held, must be obtained within 90 days of hire
• Minimum 2 years of hands-on OASIS completion or auditing experience
• Background in nursing, PT, OT, SLP, MDS, or health information management
• ICD-10 coding experience strongly preferred
• Strong analytical skills — you'll be reviewing charts for inconsistencies others miss

Nice to Have

• QAPI, Lean, or Six Sigma exposure
• Prior experience training clinicians on documentation improvement

Education

• High school diploma or GED required; Associate's degree preferred
• HCS-O or COS-C certification preferred at hire; required within 90 days if not already held

Other Requirements

• Strong written/verbal communication for staff feedback and training
• Proficiency in Microsoft Office and ability to learn new software quickly
• Highly organized, self-directed, comfortable managing multiple priorities
