Major Responsibilities:

Designs and directs the work of assigned Social Health Operations areas, including referral operations support, CHW workflow infrastructure, operational education, community resource connectivity, and implementation support.

Selects, coaches, and develops team members, sets objectives aligned with enterprise Social Health priorities, manages performance, supports succession planning, and creates processes for feedback, recognition, and performance improvement. 

Serves as the enterprise operational lead for Social Health initiatives by coordinating activities across Social Health Operations, Social Health Interventions, Community Health Worker programs, virtual care functions, clinical screening efforts, and other enterprise stakeholders to ensure alignment, accountability, communication, and successful execution of enterprise priorities. 

Translates enterprise Social Health strategy into functional operating plans, standards, workflows, playbooks, and adoption plans that support consistent execution across state divisions and markets. 

Leads governance and performance routines for assigned operational domains, including issue escalation, decision tracking, implementation readiness, standard work maintenance, and progress reporting against enterprise goals. 

Develops and coordinates enterprise reporting, executive updates, governance materials, and performance reviews related to Social Health priorities, ensuring leaders have visibility into key initiatives, risks, progress, outcomes, and operational needs. 

Partners with Information Technology, Clinical Informatics, Analytics, Community Health, Care Management, state division leaders, operations, training teams, and vendors to implement and optimize Social Health workflows, referral tools, resource directories, and closed-loop referral processes. 

Coordinates enterprise Social Drivers of Health screening and referral initiatives by partnering with clinical, operational, population health, care management, informatics, and Social Health leaders to support implementation, alignment, performance monitoring, issue resolution, executive reporting, and continuous improvement across the organization. 

Directs operational improvement initiatives to increase referral completion, strengthen closed-loop performance, improve resource visibility, reduce workflow variation, and address barriers identified through data, teammate feedback, and stakeholder input. 

Oversees development and delivery of education, onboarding, super-user structures, communications, and change management resources that support adoption of Social Health tools and workflows across assigned areas. 

Leads enterprise communication and stakeholder engagement efforts that promote awareness of Social Health programs, screening and referral initiatives, operational priorities, enterprise expectations, and key program outcomes across clinical, operational, and executive stakeholder groups. 

Develops and/or recommends operating and capital budgets for assigned areas of responsibility and controls expenditures within approved budget objectives. 

Responsible for understanding and adhering to the organization’s Code of Ethical Conduct and for ensuring that people actions, and the actions of employees supervised, comply with the policies, regulations, and laws applicable to the organization’s business. 

Education

Bachelor’s degree in healthcare administration, public health, social work, business administration, health services, or related field. 

Certification / Registration / License

None required. 

Work Experience Typically requires seven or more years of experience in healthcare operations, population health, social care, community health, care management, health equity, or related field, including three or more years of management-level experience and responsibility for complex operational initiatives. 

Knowledge / Skills / Abilities

Knowledge of social drivers of health, referral operations, CHW workflow infrastructure, community resource networks, healthcare operations, performance improvement, change management, stakeholder engagement, data-informed decision making, team leadership, and project or program management. Ability to translate strategy into operational plans, lead through influence, manage competing priorities, and drive consistent execution across a matrixed environment. 

Physical Requirements and Working Conditions:

Operates in a professional office and healthcare environment. Requires prolonged periods of sitting, computer use, virtual meetings, and communication with internal and external stakeholders. Periodic travel to system, division, market, and community locations may be required.
