As a Patient Care Coordinator, you will:
Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company's services.
Conduct facility visits at the physicians' request to assist program clinical team in determining eligibility.
Participate in weekly business development meetings with program staff and sales teams.
Assist program in processing of physician orders.
As part of our sales team, you will report to a Market Executive of Sales

Required/Preferred Experience/Skills:
RN/LPN/LVN license.
3 years nursing experience is preferred.
Bachelor of Science in Nursing is preferred.
Strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
Demonstrated strengths in customer service, account development, organization and communication
Travel within an assigned territory and to sales meetings as required.
This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
