Job Description Summary
Provides comprehensive advanced practice primary care services for patients across the lifespan in ambulatory and community-based care settings. Evaluates, diagnoses, and manages acute and chronic medical conditions through evidence-based assessment, preventive care, health screenings, medication management, patient education, and long-term disease management. Promotes wellness and disease prevention while collaborating with physicians and interdisciplinary care teams to improve health outcomes, enhance care coordination, and deliver high-quality, patient-centered primary care.The Nurse Practitioner, General / Primary Care practices independently within the advanced practice provider scope, making clinical decisions for routine and moderately complex cases and escalating beyond-scope matters to collaborating physicians. Decisions are made with minimal supervision under the physician collaboration model. This job applies Master's-level clinical training to deliver patient care within the defined APP scope and may oversee nursing and allied support staff.
How will you make an impact & Requirements
KEY RESPONSIBILITIES
Evaluate, diagnose, and manage patients presenting with acute illnesses, chronic diseases, and preventive healthcare needs through comprehensive history, physical examination, diagnostic interpretation, and evidence-based clinical decision-making.
Develop individualized treatment plans that include medication management, preventive services, lifestyle counseling, health promotion, immunizations, and ongoing monitoring of treatment outcomes.
Diagnose and manage common primary care conditions including hypertension, diabetes mellitus, hyperlipidemia, asthma, chronic obstructive pulmonary disease, obesity, thyroid disorders, depression, anxiety, musculoskeletal conditions, infectious diseases, and other acute and chronic illnesses within the advanced practice provider scope.
Order, perform, and interpret laboratory studies, diagnostic imaging, point-of-care testing, electrocardiograms, and other diagnostic evaluations within the scope of practice to guide patient care.
Perform office-based procedures within the scope of practice, including minor wound care, skin lesion management, joint injections, incision and drainage, and other approved procedures consistent with organizational policies and state regulations.
Conduct annual wellness visits, preventive screenings, health risk assessments, chronic care management, transitional care management, and age-appropriate preventive services in accordance with evidence-based clinical guidelines.
Recognize changes in patient condition, identify complex clinical presentations requiring physician consultation, and coordinate referrals to specialty care, behavioral health, community resources, and ancillary services when clinically indicated.
Collaborate with physicians, nurses, pharmacists, care managers, behavioral health providers, social workers, and other healthcare professionals to coordinate comprehensive patient care across the continuum.
Educate patients and caregivers regarding diagnoses, medications, preventive health measures, nutrition, exercise, chronic disease self-management, and treatment plans to promote informed decision-making and improved health outcomes.
Document patient encounters, medical decision-making, procedures, treatment plans, quality measures, and clinical outcomes accurately within the electronic health record while maintaining compliance with organizational standards, regulatory requirements, payer documentation guidelines, and value-based care initiatives.
Participate in quality improvement initiatives, population health programs, patient safety activities, clinical pathway development, and continuing professional development to support excellence in primary care delivery.
QUALIFICATIONS
Master's or Doctor of Nursing Practice degree from an accredited Nurse Practitioner program required.
Current, unrestricted Nurse Practitioner license and Advanced Practice Registered Nurse licensure in the applicable state required.
National board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult-Gerontology Acute Care Nurse Practitioner (AGACNP), or other certification appropriate to the practice setting required.
Current Drug Enforcement Administration registration and Basic Life Support certification required. Advanced Cardiovascular Life Support certification is preferred based on clinical assignment.
Typically requires two or more years of advanced practice clinical experience in primary care, family medicine, internal medicine, community health, or a closely related ambulatory practice. Primary care experience is preferred.
Demonstrated knowledge of primary care medicine, preventive health guidelines, chronic disease management, population health principles, evidence-based clinical practice, and ambulatory care operations.
Experience using electronic health records, clinical decision support tools, population health platforms, and healthcare technology to support quality patient care, regulatory compliance, and value-based care initiatives.
Strong clinical judgment, diagnostic reasoning, communication, patient education, interdisciplinary collaboration, cultural competence, and relationship management skills with the ability to provide compassionate, comprehensive, high-quality primary care across diverse patient populations.

$108,466.00to
$162,698.00
The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
