Starting Salary Range: $57,437 + Office Benefit Package (Parking/Remote Expenses): $180/Month subject to change

This position will provide professional, quality program services to the individuals served by the Section 811 Program. The Mental Health Practitioner will provide Housing Support Services to people with serious mental illness and/or substance use disorders that are enrolled in the Section 811 program in the Metro area. The primary function of the practitioner is to provide outreach, housing transition, and short-term housing sustaining services, with the goal of connecting the person to ongoing services.   

Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm, 40 hours per week

**This position works onsite at the RADIAS Health downtown Saint Paul office**

Duties and Responsibilities:

  • Engage in coordinated outreach efforts with RADIAS Health PATH team members to build connections and establish rapport with individuals, once approved for Section 811.
  • Utilize Housing Stabilization Services funding for transition services when possible.  These transition services will include assisting with providing the property with needed applications and verifications, helping the participant tour the apartment, assisting the applicant with setting up utilities, setting up meetings with the property, coordinating the lease signing with the property, helping the participant understand the terms of the lease, and coordinating the move.
  • Provide the purchasing and payment of items related to establishing the participant in the apartment using Housing Stabilization Moving Expense funds (e.g. rent and utility deposits, 1st month's rent, paying past due rents or utilities, essential furnishings, household supplies, and moving expenses).
  • Ensure that the person served has participated in the selection of household furnishings and supplies.
  • Provide tenancy supports to HUD Section 811 Project Based program participants who are experiencing or who have experienced long term homelessness and serious mental illness in the seven-county metro area.
  • Assist providing property with needed applications and verifications.
  • Provide instruction and monitoring on the identified service areas indicated on the client’s Housing Focused Person-Centered Plan and other assessments.
  • Maintain records of progress for each face-to-face meeting and telephone contact with client
  • Decide on appropriate interventions or assist the client in obtaining and maintaining the highest level of independence possible.
  • Ensure standards are met according to State and County guidelines and ensure appropriate reporting is submitted to Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other agencies/entities are made when necessary.
  • Provide post-transition services to assist the program participant in sustaining housing ad living successfully in the community, including making appropriate referrals for long-term support service which may include Targeted Case Management, ARMHS, CADI, ongoing HSS, etc.
  • Work with the person serviced and the Property Manager, if needed, to solve identified problems in a collaborative manner.
  • Provide problem mitigation services for 811 participants who are at risk of losing housing.
  • Coordinate and cooperate with other agency and community providers to provide the best possible care for the individual, including but not limited to landlords and other community resources.  Practitioner will complete all necessary Release of Information and documents for communication with other providers and keep these current for each person served.
  • Assist clients to create a plan to work towards service goals in regular meetings with the client.
  • Schedule appointments to see the person to assist in requested services for time and frequency agreed upon in the person's support plan.
  • Bill for services by completing documentation as required by program policies and procedures.
  • Ensure that the persons served have access to information about tenant rights and responsibilities and provide ongoing guidance to individuals in relation to adhering to the terms of the lease.
  • Must be able to transport clients in the community.
  • Attend weekly meetings with DHS, AEON, staff meetings and other meetings as scheduled.

Requirements

  • Bachelor’s degree in behavioral science or related field; and
  • 2,000 hours of supervised experience in the delivery of clinical services in the treatment of adults with mental illness.  The practitioner's experience must have been supervised, but not necessarily by a mental health professional; or
  • Completed 6,000 hours of supervised experience in the delivery of clinical services in the treatment of mental illness.

Preferred:

  • Bachelor’s degree in behavioral sciences or related fields
  • Experience providing outreach services to persons experiencing homelessness and knowledge of services/resources in Ramsey County.
