We're hiring compassionate Home Health Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) in Macomb, Wayne and Washtenaw counties to provide personalized, one-on-one care that helps patients regain strength, improve mobility, reduce pain, and safely return to the activities they love. π You Might Be a Great Fit If You:
- Conduct comprehensive physical therapy evaluations to assess functional needs, develop individualized treatment plans, provide evidence-based interventions, and monitor progress to maximize mobility, safety, and independence.
- Deliver skilled physical therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
- Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
- Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize mobility, functional outcomes, and fall prevention.
- Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention, assistive devices, and individualized Home Exercise Programs (HEPs) to promote safe mobility and recovery.
- Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.
Requirements
πΌ What We're Looking For:
- Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.
- Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.
- Home health experience preferred (minimum one year).
- Strong clinical judgment, critical thinking, and problem-solving skills.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Reliable transportation, a valid driver's license, and current auto insurance.
