About the role
Apollo Home Healthcare is seeking a Speech–Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa County, Alameda County, and the Tri-Valley area. This is a W-2 per diem role ideal for SLPs looking for schedule flexibility, autonomy, and meaningful patient relationships. You’ll provide high-quality, one-on-one care to patients in their homes while collaborating with a supportive interdisciplinary team.

What You’ll Do
As a Home Health SLP, you will:
- Perform speech-language and swallowing evaluations in patients’ homes
- Develop and implement individualized Plans of Care
- Provide skilled speech therapy interventions
- Recommend communication devices and adaptive equipment as needed
- Assess home environments for safety and functional communication needs
- Collaborate with physicians, nurses, PT/OT, and care teams
- Participate in case conferences as appropriate
- Complete timely, compliant clinical documentation
- Educate patients and caregivers on exercises and compensatory strategies
- Supervise home health aides when applicable to the plan of care
Requirements
- Active California Speech–Language Pathologist license
- Master’s degree in Speech-Language Pathology
- Current CPR certification
- Home health experience preferred (but not required)
- Strong documentation and communication skills
- Ability to work independently in the field
- Reliable transportation and valid driver’s license
