About the role
BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a part-time Outpatient at Home opportunity serving adult and geriatric clients throughout Atlantic County, NJ in Senior Living Communities.
This unique role combines the clinical focus of outpatient therapy with the flexibility and personalized care of home-based visits. You'll work one-on-one with patients in senior living settings, helping them regain strength, improve mobility, and maintain independence in the comfort of their own environment.

What You'll Do
- Provide outpatient physical therapy services in designated facilities settings
- Perform comprehensive physical therapy evaluations and reassessments
- Assess mobility, strength, balance, gait, transfers, endurance, and overall safety
- Develop individualized treatment plans designed to maximize function and independence
- Implement therapeutic exercise programs, gait training, balance training, and other evidence-based interventions
- Collaborate with nurses and other healthcare professionals to coordinate patient care
- Educate patients and caregivers on exercise programs, fall prevention, safety strategies, and disease management
- Document patient care using a user-friendly electronic medical record system

Qualifications
- Current New Jersey Physical Therapist (PT) license
- Graduation from a program approved by the American Physical Therapy Association (APTA) or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred

Additional Information
Base Pay:
$48.00 - $53.00 per hour, depending upon qualifications and subject to negotiation.
