Role Scope:
We are looking for a compassionate, detail-oriented Licensed Vocational Nurse (LVN) to join our interdisciplinary team at our PACE (Program of All-Inclusive Care for the Elderly) center. The LVN plays a critical role in delivering high-quality, person-centered care to our participants—primarily frail seniors—within the day center and clinic setting. Under the supervision of an RN, the LVN provides nursing care, assists with clinic operations, performs triage, and supports health and wellness activities while contributing to our engagement-focused care environment that supports both participant outcomes and organizational sustainability.
Core Responsibilities
Clinical Care Coordination
- Provide direct nursing care and treatments to PACE participants as directed by the RN and/or physician
- Administer medications and injections, monitor vital signs, and observe participant health status
- Conduct triage of participants presenting with acute or urgent needs, using standardized protocols to prioritize care and alert the RN or provider as needed
- Perform assessments and document participant conditions, responses to treatments, and changes in health status
- Assist with intake evaluations, wound care, and chronic disease management under clinical protocols
- Implement care plan interventions with attention to both clinical outcomes and participant engagement
- Support the management of participant caseloads in collaboration with RN Case Managers
- Coordinate care transitions between home, day center, and other care settings
- Support infection control and safety protocols in the clinic and day center environment
- Perform other duties as assigned
Documentation and Regulatory Compliance
- Maintain accurate and timely documentation in electronic health records (EHR)
- Document participant assessments, interventions, and responses according to established protocols
- Ensure all nursing documentation supports eligibility criteria and demonstrates medical necessity
- Participate in continuous quality improvement initiatives related to documentation excellence
- Adhere to all PACE regulations, policies, and procedures while maintaining participant dignity
Interdisciplinary Collaboration
- Contribute to Interdisciplinary Team (IDT) approach, assisting with participant assessments, care delivery, and communication with other team members as well as to quality improvement activities
- Collaborate effectively with RNCMs, physicians, therapists, social workers, and other team members
- Support the integration of participant preferences into the clinical care approach
- Facilitate communication between participants, caregivers, and the interdisciplinary team
- Contribute to care plan development and implementation from a nursing perspective
- Coordinate care with external providers and services as needed (e.g., specialty appointments, DME vendors)
Participant Engagement
- Build therapeutic relationships with participants that support both clinical outcomes and quality of life
- Implement engagement strategies that enhance participant well-being
- Educate participants and caregivers on medication management, nutrition, hygiene, and other health topics
- Support participant autonomy and decision-making while addressing safety considerations
- Contribute to a culture of compassionate, person-centered care across all interactions
Qualifications:
- Current California LVN license in good standing
- Minimum of 1 year nursing experience, preferably in geriatrics, home health, or adult day health care
- CPR/BLS certification required
- Strong clinical, organizational, and communication skills
- Experience with electronic medical records (EMR)
- Compassionate, respectful, and committed to the care of older adults
- Ability to work in various care settings (clinic, day center, participant homes)
- Valid driver's license and reliable transportation
- Aligns with our purpose and our values, and is excited about living those out in daily practice
- Ability to lead with influence, especially for cross-functional roles
Nice to have:
- Previous PACE program experience
- Geriatric certification or specialized training
- Experience with interdisciplinary team models
- Bilingual skills (e.g., Spanish, Cantonese, Tagalog)
- Familiarity with value-based care models
Compensation:
We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $29 - $33 per hour and is a bonus-eligible position. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
