About the Role:
As a Telehealth Coordinator, you will serve as the primary point of contact for patients throughout their home health journey. You will provide clinical support, proactive patient outreach, care coordination, and healthcare navigation while partnering with branch clinicians, physicians, caregivers, and interdisciplinary teams to improve patient outcomes.

This role combines inbound clinical support with proactive patient engagement to reduce avoidable hospitalizations, improve patient satisfaction, identify barriers to care, and ensure timely communication across the care continuum. Your work allows field clinicians to spend more time providing direct patient care while ensuring every patient receives compassionate, coordinated support.

Responsibilities:
-Answer inbound patient, caregiver, and referral calls while providing telephone assessment, education, and appropriate clinical triage.
-Conduct proactive outreach including welcome calls, high-risk monitoring, tuck-in calls, transition-of-care follow-up, post-hospital outreach, and missed visit follow-up.
-Assess patient concerns and identify clinical and non-clinical barriers to receiving care.
-Coordinate communication between patients, physicians, branch staff, scheduling teams, and interdisciplinary care teams.
-Reinforce patient education using approved clinical resources and individualized care plans.
-Escalate urgent clinical concerns according to established protocols.
-Document all patient interactions accurately in Homecare Homebase (HCHB) and other required systems.
-Monitor trends that impact patient outcomes and recommend workflow improvements.
-Participate in quality improvement initiatives, team huddles, competency development, and ongoing education.
-Deliver compassionate, patient-centered service while maintaining HIPAA compliance and Medicare regulatory standards.

Qualifications:
-Minimum of two years of Home Health experience.
-Experience performing patient education, or care coordination.
-Strong understanding of Medicare Conditions of Participation and home health eligibility requirements.
-Excellent communication, critical thinking, and problem-solving skills.
-Ability to prioritize multiple patients and tasks in a fast-paced remote environment.
-Experience documenting in electronic medical record systems.
-Proficiency with Microsoft Office applications.
-Reliable internet connection and dedicated home office workspace.

Preferred:
-Experience using Homecare Homebase (HCHB).
-Previous telehealth or healthcare contact center experience.
-Experience supporting multiple home health branches or geographically dispersed clinical teams.
-Knowledge of quality improvement, workflow optimization, and patient engagement strategies.
-Bilingual (English/Spanish).
-Experience with transition of care, chronic disease management, or centralized healthcare operations.
