The Population Health Program Manager is accountable for the success of assigned population health programs and initiatives serving Medicaid and Medicare members. This role translates population health strategy into operational execution by leading the design, implementation, and evaluation of internal/external programs, services, and processes that address the needs of defined populations and advance organizational goals related to health outcomes, equity, quality, cost, utilization, and member experience.

This is a hybrid position with the expectation to be in office 1-2x per month.

Estimated Hiring Range:

$102,330.00 - $125,070.00

Bonus Target:

Bonus - SIP Target, 5% Annual

Essential Responsibilities

  • Lead the design, implementation, and ongoing management of evidence-based population health programs, interventions, and processes leveraging internal resources and external partners to meet defined objectives.
  • Provide matrix leadership to coordinate population health initiatives across internal departments and teams, aligning scope, timelines, roles, and deliverables.
  • Define program success measures and monitor performance using population health, quality, utilization, and experience data; evaluate program effectiveness; identify gaps; and lead continuous improvement activities in partnership with analytics and operational teams.
  • Develop and implement strategies to engage members, providers, and community partners to support program adoption, participation, and adherence, including development of operational workflows and communication plans.
  • Prepare and present routine and ad hoc status updates, performance summaries, and recommendations to leadership and governance forums.
  • Ensure programs align with organizational priorities, regulatory requirements, and best practices.
  • Identify and support partnerships across physical health, behavioral health, oral health, social health, and community organizations.
  • Collaborate across organizational departments and lines of business to align program priorities and improvement actions.
  • Provide technical assistance and training to internal teams and external partners.
  • Maintain awareness of applicable regulations, accreditation requirements, and emerging best practices in population health management; assess impact and recommend updates to programs, workflows, and measures.
  • Represent the organization on program-specific local, state, or national workgroups and committees.
  • Develop and manage project plans, including timelines, milestones, and KPIs.
  • Apply quality improvement and project management tools to improve program performance.

Experience and/or Education

Required

  • Minimum 6 years of experience in population health, public health, health care management, quality improvement, or a related field
  • At least 2 years in project or program management
  • Demonstrated matrix leadership (no direct reports required), including leading projects/work groups, influencing stakeholders, and driving accountability for deliverables
  • Experience leading cross-functional teams and managing complex initiatives
  • Experience achieving impact through program or project management
  • Experience using population health data to drive decisions and action

Preferred

  • Master’s degree in public health, healthcare administration, business, or related field
  • Experience in managed care, CCOs, or population health/quality programs
  • Experience implementing innovative or pilot programs and scaling effective practices

Knowledge, Skills and Abilities Required

Knowledge

  • Strong expertise in Medicaid and Medicare programs, regulations, and managed care operations
  • Expertise in population health strategies, value-based care, and quality improvement
  • Advanced knowledge in quality improvement methodologies and health care performance metrics (HEDIS, NCQA, CMS Star Rating, CCO Metrics)
  • Expertise in program design, implementation, and monitoring
  • Knowledge of healthcare delivery systems, care management operations, social determinants of health, and community resources

Skills and Abilities

  • Proven ability to lead through influence, facilitate cross-functional decision-making, and drive accountability for deliverables
  • Proficiency in interpreting and analyzing healthcare data from dashboards and reports (e.g., Tableau, Power BI, Excel, or similar tools)
  • Advanced Excel functions for manipulating, analyzing, and visualizing population and claims data to inform data-driven decision-making and actionable recommendations
  • Experience in program evaluation, cost analysis, and identifying trends in healthcare utilization
  • Strong leadership and team management skills with the ability to oversee large-scale initiatives
  • Excellent project management skills, including experience with Agile methodologies and performance tracking
  • Proven ability to build and maintain relationships with healthcare providers, community organizations, and government agencies
  • Effective communication and executive presentation skills, with the ability to translate complex data into actionable insights
  • Ability to manage competing priorities and deliver results in a fast-paced, highly collaborative environment
  • Strong analytical and problem-solving skills; ability to synthesize complex data into clear insights and actionable recommendations
  • Experience with quality improvement methodologies and other process optimization frameworks
  • Ability to lead independently in a fast-paced environment, maintaining alignment between complex programs and organizational strategy
  • Experience developing policies/procedures, facilitating governance forums, or managing vendors/contracts
  • Ability to work effectively with diverse individuals and groups
  • Ability to learn, focus, understand, and evaluate information and determine appropriate actions
  • Ability to accept direction and feedback, as well as tolerate and manage stress
  • Ability to see, read, and perform repetitive finger and wrist movement for at least 6 hours/day
  • Ability to hear and speak clearly for at least 3-6 hours/day

Work Location: Hybrid-Office 1-2 days/month
