Our newly renovated, state-of-the-art therapy space and specialized programs provide an excellent environment to help patients regain strength, mobility, independence, and confidence.
Position Details
Position: Physical Therapist (PT)
Location: Vasona Creek Healthcare – Los Gatos/South Bay Area
Pay Rate: $57.00–$65.00+ per hour, depending on experience
Opportunities: Full-Time, Part-Time & PRN

Experienced & Collaborative Team
Work alongside a multidisciplinary rehabilitation team of PTs, PTAs, OTs, and SLPs, supported by clinicians with 10+ years of experience. Our team values collaboration, clinical growth, and mutual respect.
Your Role & Impact
As a Physical Therapist, you will play an essential role in helping residents achieve their highest level of function and independence.
Responsibilities include:
Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
Provide skilled therapy services to short-term rehabilitation and long-term care residents.
Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management.
Monitor patient progress and modify treatment plans as appropriate.
Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.
Educate patients and families regarding treatment plans, exercises, mobility, and strategies for continued progress.
Promote safe mobility, functional independence, and successful discharge planning.
Maintain accurate and timely clinical documentation.
Advocate for patients and support their goals throughout the rehabilitation process.

Qualifications
Current California Physical Therapist (PT) license required.
Previous experience in skilled nursing, post-acute, or rehabilitation settings preferred.
Strong clinical reasoning and communication skills.
Passion for rehabilitation and helping patients achieve meaningful functional outcomes.
Commitment to evidence-based, patient-centered care.
Ability to work collaboratively as part of a multidisciplinary rehabilitation team.
