What You’ll Do

✔ Assess patients’ psychosocial, financial, and environmental needs
✔ Complete initial and ongoing social work evaluations
✔ Support care planning and discharge planning
✔ Connect patients and families with community resources
✔ Communicate changes and concerns with care team and physicians
✔ Document visits and maintain compliance with care standards
✔ Provide counseling and guidance to patients and caregivers

What You Need

✔ Master’s or Doctoral Degree in Social Work (CSWE accredited)
✔ Active Social Worker license
✔ 1+ year of healthcare social work experience
✔ Strong communication and interpersonal skills
✔ Ability to travel within the coverage area (50%)
✔ Valid driver’s license, auto insurance, and reliable transportation
✔ Ability to lift, push, or pull up to 50–100 lbs
