Gritter Francona is looking for a Project Manager that will provide comprehensive project management and implementation support for the design, launch, management, evaluation, and preparation for scaling an Elevated Risk Care Management (ERCM) Pilot focused on improving the coordination, effectiveness, and continuity of suicide prevention-related care for Veterans at elevated risk.

The Project Manager will serve as the single contractor point of contact for the Veterans Health Administration (VHA) for requests for subject matter expert (SME) support and matters related to contractor performance. The Project Manager will work closely with Government program leads, designated stakeholders, clinical teams, and contractor personnel to coordinate and execute approved activities throughout the pilot lifecycle, including Concept, Explore, Design, Implement, Evaluate, and Spread phases.

The position requires strong project management, stakeholder coordination, healthcare implementation, clinical workflow, documentation, and communication skills. The Project Manager will ensure that project activities remain organized, on schedule, appropriately documented, and aligned with Government objectives and requirements.

Key Responsibilities

Project Management and Coordination

  • Serve as the primary contractor point of contact to VHA for project-related matters, SME support requests, deliverables, schedules, risks, issues, and contractor performance.
  • Plan, initiate, manage, execute, monitor, and close out project activities in accordance with approved requirements and timelines.
  • Develop and maintain comprehensive project plans, schedules, milestones, dependencies, deliverables, and action-item tracking.
  • Coordinate multiple project activities and workstreams across Government and contractor teams.
  • Monitor project progress and proactively identify risks, issues, resource constraints, dependencies, and potential schedule impacts.
  • Develop and implement mitigation and corrective-action strategies.
  • Ensure project deliverables are completed accurately, timely, and in accordance with Government requirements.
  • Maintain organized and current project documentation, decision logs, risk and issue registers, meeting records, and other project management artifacts.
  • Support project governance and coordination through Integrated Project Team (IPT) meetings and working groups.

Concept and Explore Phase

  • Support IPT formation and establish effective project coordination processes.
  • Assist Government program leads in defining the problem statement, project objectives, hypothesis, scope, assumptions, and desired outcomes.
  • Support development of the business, clinical, and operational foundation for the ERCM Pilot.
  • Assist with development of governance structures, project metrics, reporting requirements, and pilot site-selection criteria.
  • Coordinate stakeholder input and document decisions, requirements, dependencies, and action items.
  • Prepare project documentation and briefing materials for leadership review and decision-making.

Design Phase

  • Support development and finalization of the ERCM operating model and associated project implementation plans.
  • Coordinate development of clinical and operational workflows designed to improve care coordination for Veterans at elevated risk.
  • Support integration and coordination of follow-up activities for Veterans identified through multiple pathways, including REACH VET, Risk ID, PDE, SPED, post-event outreach, and Veterans Crisis Line-related requests.
  • Help identify opportunities to reduce redundant outreach and improve coordination among Behavioral Health Interdisciplinary Program (BHIP) teams, Mental Health Treatment Coordinators (MHTCs), and other clinical teams.
  • Coordinate development of standard operating procedures, workflow documentation, implementation materials, training materials, and other project resources.
  • Support pilot-site readiness and implementation planning.

Implementation and Pilot Operations

  • Coordinate implementation of the ERCM Pilot across three selected sites.
  • Develop and maintain site-specific project schedules, implementation plans, and readiness activities.
  • Coordinate onboarding, training, stakeholder engagement, workflow implementation, and project launch activities.
  • Track implementation progress against established milestones and performance requirements.
  • Facilitate communication between pilot sites, Government program leads, contractor personnel, and clinical stakeholders.
  • Monitor project performance through standardized reports, dashboards, status updates, and performance measures.
  • Identify implementation barriers and coordinate resolution with appropriate stakeholders.
  • Track and communicate project risks, issues, decisions, dependencies, and corrective actions.
  • Ensure lessons learned and site-specific implementation experiences are documented throughout the pilot.

Evaluation and Reporting

  • Support development and execution of project evaluation activities.
  • Coordinate collection and organization of project and implementation data in collaboration with Government stakeholders and evaluation personnel.
  • Monitor project performance against established measures, milestones, and objectives.
  • Support preparation of dashboards, status reports, executive briefings, and other performance-monitoring products.
  • Assist with identification of trends, lessons learned, implementation challenges, and opportunities for improvement.
  • Support preparation of an evidence-based assessment of pilot effectiveness.
  • Translate project findings into actionable information for Government leadership and program stakeholders.

Stakeholder and SME Coordination

  • Serve as the central coordination point for VHA requests for contractor SME support.
  • Coordinate the availability and activities of contractor SMEs in response to Government requirements.
  • Facilitate meetings, working sessions, IPT activities, and stakeholder engagements.
  • Develop agendas, meeting materials, minutes, action-item trackers, and decision documentation.
  • Maintain effective working relationships with Government program leads, clinical stakeholders, pilot-site personnel, and contractor team members.
  • Coordinate communication across multidisciplinary teams and ensure stakeholder concerns and requirements are appropriately addressed.
  • Escalate significant risks, issues, resource requirements, or performance concerns to appropriate Government and contractor leadership.

Technical Writing and Documentation

  • Prepare clear, accurate, and professional project and technical documentation.
  • Develop project plans, status reports, implementation plans, meeting materials, briefings, workflow documentation, training materials, evaluation summaries, action plans, and transition documents.
  • Prepare materials appropriate for executive review, field implementation, and operational use.
  • Ensure project documentation is maintained in an organized, consistent, and accessible manner.
  • Support development of standardized templates, tools, and processes that facilitate implementation and future expansion.

Transition and Scale Planning

  • Support development of an action plan based on pilot evaluation findings and lessons learned.
  • Coordinate development of transition materials and implementation resources for phased national expansion.
  • Support development of implementation toolkits, training resources, communication materials, workflow guidance, and performance-monitoring tools.
  • Assist with planning a deliberate national rollout beginning with operational environments similar to the pilot sites and expanding incrementally.
  • Identify project dependencies, resource requirements, risks, and readiness considerations associated with expansion.
  • Support ongoing performance monitoring and continuous improvement during transition and scale-up.

Requirements

  • Bachelor’s degree (BA/BS) from an accredited institution in business administration, healthcare administration, health services, public health, project management, or a related field.
  • Minimum of four years of relevant project/program management experience.
  • Project Management Professional (PMP) certification.
  • Preferred experience supporting the Veterans Health Administration (VHA, Department of Veterans Affairs (VA), or another large integrated healthcare system.
