As a Care Coordinator you are the Communication Hub, the Emotional Support Co-Worker, and the Air-Traffic Controller for our nurses and caregivers in flight.  If you want respect, appreciation, and significance in your work life, consider applying for our open Care Coordinator position. What You Will Do

Coordinate daily caregiver schedules based on client care plans and staff availability
Support and communicate with caregivers working in the field

Complete client openings in the field
Respond to client and employee questions or concerns and assist with resolution
Maintain accurate documentation, records, and reports
Assist with quality initiatives, staff communication, and training support
Provide professional and courteous communication with clients, families, and team members

What We Are Looking For

Must be at least 18 years of age
High school diploma or equivalent required
Previous office or administrative experience required
Strong communication and organizational skills
Ability to multitask and stay organized in a fast-paced environment
Professional, dependable, and team-oriented attitude
