About the Role
Help patients overcome the social and emotional challenges that impact their ability to recover and remain independent at home. As a Home Health Medical Social Worker, you’ll provide compassionate psychosocial support that helps patients and families navigate illness, chronic conditions, and barriers to successful recovery. Through assessments, counseling, education, and resource coordination, you’ll help patients access the support, tools, and resources they need while ensuring their emotional, social, and environmental needs are addressed as part of their overall care plan.
This role is ideal for a licensed MSW who is passionate about making a meaningful impact through whole-person care. You’ll collaborate with nurses, therapists, physicians, and caregivers to develop individualized care plans, connect patients with community resources, identify barriers to recovery, and support patient and caregiver independence. Your clinical expertise, communication skills, and commitment to compassionate service will help improve outcomes, strengthen care coordination, and enhance the healthcare experience for patients and families.
Location
- Office Location: 801 Rosehill Road, Jackson, MI 49202
- Main Service Area: Jackson
- This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes
Hours
- Monday to Friday, 8 a.m. to 5 p.m.
Required Qualifications
- Master's or doctoral degree from a CSWE-accredited school of social work
- Current State License as a Master's Social Worker (MSW/LMSW/LCSW)
- Minimum 1 year of experience as a licensed Master's Social Worker
- Reliable transportation with valid automobile insurance
- Willingness to travel daily for home visits within service area
Preferred Qualifications
- Previous home health or medical social work experience
- Knowledge of Medicare home health benefit and Conditions of Participation
- Experience with care coordination and discharge planning
