About this Transitional Care Manager role at BAYADA Home Health Care

BAYADA Home Health Care is currently seeking an experienced clinician or Social Worker to fill the position of Transitional Care Manager (TCM) in our
Indianapolis, Indiana territory. TCMs/Clinical Liaisons at BAYADA interact with discharge planners, case managers, and clients and families at designated facilities to coordinate the initiation of home health care services.
Position Summary:
- Build relationships. Develop and sustain strong partnerships with referral sources, physicians, and other health care professionals, in keeping with The BAYADA Way.
- Tell our story. Clearly explain the features and benefits of BAYADA services to referral partners and clients, and grow our business through those relationships.
- Determine eligibility. Apply your knowledge of clinical and regulatory home care eligibility criteria to every referral.
- Make the handoff seamless. Coordinate with the service office to set up the physician-ordered services quickly and accurately.
- Stay organized.

We’re looking for a motivated advocate who is driven to help patients get the right care, at the right time, in the right setting. You’re a self-starter who is hungry, humble, and smart: you take initiative, you put the team and the client ahead of yourself, and you read people and situations well.
Required
- Motivated, self-starting mindset with a passion for advocating for patients
- Hungry, humble, and smart
- Proven strength in interpersonal skills
- Working knowledge of the Medicare Conditions of Participation (COPs) and other applicable program requirements
- At least one year of health care experience
Preferred
- Experience in home health care or hospice
- Previous discharge planning experience
- A sales or business development background

BAYADA Offers:
- Base Salary: $70,000 to $80,000.00 / year depending on experience
