Sunnyvale Post Acute is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities for an SLP who is passionate about helping residents improve communication, swallowing, and overall quality of life.

Pay Rate: $60.00–$65.00 per hour, DOE
Position Types: Full-Time, Part-Time & PRN

Position Overview

As an SLP at Sunnyvale Post Acute, you will provide individualized evaluation and treatment to residents with communication, cognitive, and swallowing needs. You will work closely with residents, families, nursing staff, physicians, and the interdisciplinary rehabilitation team to develop and implement effective treatment plans.

Key Responsibilities

  • Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.

  • Provide skilled therapy services based on resident needs and established care plans.

  • Monitor resident progress and modify treatment approaches as appropriate.

  • Complete accurate and timely clinical documentation.

  • Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.

  • Educate residents, families, and caregivers regarding treatment recommendations and strategies.

  • Promote resident safety, independence, communication, and quality of life.

  • Maintain compliance with applicable professional, state, and federal standards.

Qualifications

  • Current California Speech-Language Pathologist (SLP) license.

  • Appropriate certification and credentials required to practice in California.

  • Experience in skilled nursing, post-acute care, or rehabilitation preferred.

  • Strong communication, clinical, and organizational skills.

  • Compassionate, resident-centered approach.

  • Ability to work effectively as part of an interdisciplinary team.
