📋 Key Responsibilities
- Conduct comprehensive screenings, evaluations, and treatments for residents with speech, language, cognitive, and swallowing disorders.
- Develop and implement individualized, evidence-based care plans targeted at restoring maximum patient independence.
- Collaborate closely with a multidisciplinary team, including PTs, OTs, nursing staff, and physicians.
- Maintain accurate, timely documentation of patient progress using electronic health records (EHR).
- Educate and counsel patients and their families regarding treatment plans and home programs.
🔍 Qualifications & Requirements
- Master’s degree in Speech-Language Pathology from an accredited program.
- Current or eligible for State Licensure as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC-SLP) preferred, but Clinical Fellows (CFs) are highly encouraged to apply.
- SNF or geriatric rehabilitation experience is preferred, but not required.
- Strong communication, time-management, and interpersonal skills.
