Our client based in Lincolnshire is seeking experienced Social Workers to join their Mental Health Discharge Service. This is an excellent opportunity for skilled practitioners to support adults with mental health needs through timely and effective hospital discharge pathways, whilst also managing a community caseload and undertaking ongoing assessments and reviews.

The successful candidates will manage a caseload of adults requiring assessment, care and support planning, and the coordination of services to facilitate safe and effective discharge from inpatient mental health settings. Alongside discharge planning responsibilities, you will undertake Care Act assessments, reviews, safeguarding enquiries, and person-centred support planning to ensure individuals receive the right support at the right time.

Working within a multidisciplinary environment, you will collaborate closely with NHS colleagues, families, carers, community services, and care providers to promote independence, reduce delays, and achieve positive outcomes for individuals transitioning from hospital to community-based support.

What will your responsibilities be? 

  • Managing a caseload of adults with mental health needs requiring assessment, intervention, discharge planning, and ongoing community support.
  • Facilitating safe, timely, and person-centred discharges from inpatient mental health settings in line with statutory responsibilities and best practice.
  • Undertaking strengths-based Care Act Assessments to identify eligible needs, desired outcomes, and appropriate support arrangements.
  • Completing reviews of existing care and support plans to ensure services remain effective, proportionate, and responsive to changing needs.
  • Developing and implementing person-centred care and support plans that promote independence, recovery, wellbeing, and community inclusion.
  • Coordinating community services, support packages, placements, and resources to ensure successful discharge outcomes.
  • Working collaboratively with NHS professionals, hospital teams, community mental health services, providers, families, carers, and partner agencies.
  • Undertaking safeguarding enquiries and interventions in accordance with the Care Act 2014 and local safeguarding procedures.
  • Assessing and managing risk, ensuring that appropriate safeguarding measures and support arrangements are implemented where necessary.
  • Applying the Mental Capacity Act 2005 and undertaking Mental Capacity Assessments where required to support decision-making and protect individual rights.
  • Supporting individuals through periods of crisis and increased vulnerability, utilising professional judgement and available resources to achieve positive outcomes.
  • Preparing comprehensive assessments, reports, care plans, and case recordings in line with legislative and organisational requirements.
  • Participating in multi-disciplinary meetings, discharge planning meetings, and case discussions to ensure coordinated and effective service delivery.
  • Maintaining accurate, timely, and up-to-date records to support high-quality statutory social work practice.

Qualifications and Experience: 

To be successful in this role you must have;  

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Minimum of two years' post-qualified experience within Adult Social Care, Mental Health Services, Hospital Discharge, or Integrated Health and Social Care settings.
  • ental Health Experience: Experience working with adults experiencing mental ill health, complex needs, and associated risks.
  • Hospital Discharge Experience: Experience supporting discharge pathways and coordinating services to facilitate safe transitions from hospital to community settings.
  •  Legislation Knowledge: Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Human Rights Act, and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake Care Act assessments, reviews, risk assessments, support planning, and safeguarding interventions.
  • Safeguarding Experience: Experience identifying, investigating, and managing safeguarding concerns involving vulnerable adults.
  • Multi-Agency Working: Ability to work effectively within multidisciplinary teams and build strong relationships with health professionals, providers, carers, families, and partner agencies.
  • Communication Skills: Excellent communication, report-writing, analytical, organisational, and decision-making skills.
  • Autonomous Working: Ability to work independently, prioritise workload effectively, and manage competing demands within a fast-paced environment.
  • Driving Licence: A valid UK driving licence and access to a vehicle are essential. The role requires travel across Lincolnshire and occasional travel further afield as required.
