Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Hospital Discharg team, based inWest Sussex. This is an excellent opportunity for a dedicated practitioner to support adults through safe and timely hospital discharge, helping individuals maximise their independence, wellbeing and long-term outcomes following periods of hospital admission.

The successful candidate will work within a fast-paced and supportive multidisciplinary environment, undertaking strengths-based assessments, coordinating care and support arrangements and working closely with health professionals, carers, families, community services, and voluntary sector organisations. You will play a key role in supporting individuals both within acute hospital settings and following discharge into the community, ensuring appropriate support is in place to promote recovery and independence.

This role requires a compassionate and skilled Social Worker who can respond effectively within statutory timescales, manage complex situations and support people to regain skills and confidence following illness, injury or hospital admission. The team offers strong leadership, professional support, and excellent opportunities for continued development and learning.

What will your responsibilities be? 

• Delivering a statutory Adult Social Care service within hospital discharge pathways and community-based follow-up support.
• Undertaking Care Act 2014 assessments to identify eligible care and support needs.
• Completing strengths-based assessments that promote independence, recovery, and positive outcomes.
• Supporting safe and timely hospital discharge planning for adults with complex care and support needs.
• Assessing individuals both within hospital settings and following discharge into the community.
• Developing, implementing, and reviewing person-centred Care and Support Plans that maximise independence and wellbeing.
• Arranging and coordinating care packages, support services, equipment, and community resources to meet assessed needs.
•  Working collaboratively with hospital staff, health professionals, community providers, carers, families, and voluntary sector organisations.
• Supporting individuals to regain skills, confidence, and independence following hospital admission wherever possible.
• Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
• Identifying and responding to safeguarding concerns in line with Adult Safeguarding legislation and procedures.
• Assessing and managing risk effectively whilst balancing individual choice, safety, and positive risk-taking
• Maintaining accurate, timely, and legally compliant assessments, reports, case records, and support plans.
• Participating in multidisciplinary meetings, discharge planning discussions, safeguarding meetings, and case reviews.
• Working flexibly across hospital and community settings to meet service demands and deliver responsive support.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration
• Eligible to work in the UK
• Full UK driving licence
• Post-qualified experience
• Degree in Social Work (MA, BA, BSc, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification)
