Our client based in Shropshire is seeking an experienced Senior Social Worker to join their Community Team, supporting adults across the North Shropshire locality. This is an excellent opportunity for a highly skilled practitioner to work with older adults, people with physical disabilities, and individuals with complex care and support needs, delivering strengths-based and person-centred interventions that promote independence and wellbeing.

The successful candidate will manage a complex caseload whilst providing high-quality assessments, care planning, safeguarding interventions, and professional social work support. Working within Adult Social Care, you will play a key role in helping people remain safely within their own homes and communities, reducing reliance on long-term services wherever possible. You will work collaboratively with health professionals, carers, families, and partner agencies to achieve positive outcomes and support effective service delivery.

Key Responsibilities

Core duties include:

  • Undertaking Care Act 2014 assessments, reviews, and support planning for adults with disabilities, older people, and their carers.
  • Managing a complex caseload within the North Shropshire locality, ensuring statutory responsibilities are delivered to a high professional standard.
  • Applying strengths-based practice to support individuals to maximise independence, wellbeing, and community participation.
  • Developing and reviewing person-centred care and support plans that reflect individual needs, strengths, and desired outcomes.
  • Contributing to safeguarding enquiries and Multi-Agency Protection of Vulnerable Adults procedures, including investigations and risk management processes.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Working with individuals in hospital settings to facilitate appropriate, timely, and safe discharges back into the community.
  • Coordinating services and support arrangements to prevent unnecessary hospital admissions and promote independent living.
  • Working collaboratively with health colleagues, housing providers, carers, voluntary organisations, and other partner agencies to deliver integrated support.
  • Identifying, assessing, and managing risk whilst balancing individual rights, choice, and autonomy.
  • Promoting service user engagement and ensuring people, their families, and carers are involved in assessment, planning, and decision-making processes.
  • Maintaining accurate and timely case records, assessments, reports, and support plans in line with legislative and organisational requirements.

Qualifications & Experience

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England (SWE).
  • Experience: Significant post-qualified experience within Adult Social Care, including complex assessments, safeguarding, and community-based social work.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management.
  • Safeguarding Experience: Experience contributing to safeguarding investigations and multi-agency adult protection procedures.
  • Hospital Discharge Experience: Experience supporting timely and effective discharge planning and community-based support arrangements.
  • Multi-Agency Working: Strong experience working in partnership with health professionals, community services, carers, and other statutory and voluntary sector organisations.
  • Communication Skills: Excellent communication, analytical, report-writing, and decision-making skills with the ability to work autonomously and collaboratively.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
