About this Senior Social Worker – Community role at Liquid Personnel
Our client based in Shropshire is seeking an experienced Senior Social Worker to join their Community Team, supporting adults across the North Shropshire locality. This is an excellent opportunity for a highly skilled practitioner to work with older adults, people with physical disabilities, and individuals with complex care and support needs, delivering strengths-based and person-centred interventions that promote independence and wellbeing.
The successful candidate will manage a complex caseload whilst providing high-quality assessments, care planning, safeguarding interventions, and professional social work support. Working within Adult Social Care, you will play a key role in helping people remain safely within their own homes and communities, reducing reliance on long-term services wherever possible. You will work collaboratively with health professionals, carers, families, and partner agencies to achieve positive outcomes and support effective service delivery.
What will your responsibilities be?
- Undertake Care Act 2014 assessments, reviews, and support planning for adults and carers.
- Manage a complex caseload, ensuring statutory duties are completed to a high professional standard.
- Apply strengths-based practice to maximise independence, resilience, and community participation.
- Develop and review person-centred care and support plans tailored to individual needs and aspirations.
- Contribute to safeguarding enquiries and Multi-Agency Protection of Vulnerable Adults procedures.
- Undertake Mental Capacity Assessments and contribute to Best Interest decision-making in line with the Mental Capacity Act 2005.
- Support individuals in hospital settings to facilitate safe and timely discharge back into the community.
- Coordinate support services that prevent unnecessary hospital admissions and promote independent living.
- Work in partnership with health professionals, housing providers, carers, voluntary organisations, and other agencies.
- Assess and manage risk effectively while balancing individual choice, rights, and autonomy.
- Promote active involvement of service users, carers, and families in assessment and care planning processes.
- Maintain accurate case records, assessments, reports, and support plans in accordance with legislative and organisational requirements.

Qualifications and Experience:
- MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
- Registered with Social Work England (SWE).
- Significant post-qualified experience within Adult Social Care.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Experience undertaking complex assessments, reviews, support planning, and risk management.
- Experience contributing to safeguarding investigations and multi-agency adult protection procedures.
- Previous hospital discharge and community-based social work experience.
- Strong partnership-working skills with health professionals, carers, and statutory and voluntary sector organisations.
- Excellent communication, analytical, report-writing, and decision-making skills.
- Full UK driving licence and access to a vehicle is desirable.
