Liquid Personnel is recruiting an experienced Senior Practitioner – Hospital Discharge & Home First Service for its client’s Adult Social Care team based in Southampton. This is a full-time agency role offering £32 - £35 per hour and supporting safe hospital discharges, strengths-based practice, and positive outcomes for adults within the community.

The Role

Our client is seeking an experienced Senior Practitioner to join their Hospital Discharge & Home First Service. This is an excellent opportunity for an accomplished Adult Social Care professional to provide leadership and expert practice guidance within a fast-paced multidisciplinary environment.

Working closely with health professionals, housing services, community organisations, families, and carers, you will support adults to leave hospital safely, maximise independence, and remain connected to their communities. Alongside managing complex casework, you will play a key role in supervision, mentoring, quality assurance, and service improvement.

What will your responsibilities be?

As a Senior Practitioner, you will:

  • Provide professional leadership, coaching, mentoring, and practice guidance to Social Workers, Social Care Practitioners, students, and newly qualified staff.
  • Support timely, safe, and effective hospital discharges in line with Home First principles.
  • Promote strengths-based practice, Care Act assessments, safeguarding interventions, Mental Capacity Assessments, and outcome-focused support planning.
  • Support practitioners with complex casework, risk management, safeguarding concerns, and legal literacy.
  • Undertake and oversee high-quality assessments, risk analysis, and decision-making for adults with complex care and support needs.
  • Work collaboratively with NHS professionals, housing providers, community services, voluntary organisations, families, and carers.
  • Deliver reflective supervision and professional support to enhance practice and development.
  • Lead on quality assurance, audits, practice reviews, performance monitoring, and service improvement initiatives.
  • Ensure compliance with statutory responsibilities and evidence-based practice frameworks.
  • Represent the service at multidisciplinary meetings and professional forums.
  • Deputise for the Team Manager where required and support operational leadership across the service.

Qualifications and Experience

To be successful in this role, you will need:

  • MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW
  • Current Social Work England registration
  • Significant post-qualified Adult Social Care experience
  • Strong experience in hospital discharge, safeguarding, complex case management, and risk management
  • Experience providing supervision, coaching, mentoring, and professional support
  • Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and Court of Protection processes
  • Experience undertaking audits, quality assurance activity, and practice development
  • Excellent assessment, analytical, communication, report-writing, and decision-making skills
  • Ability to work effectively within a fast-paced hospital-based environment
