About this Senior Practitioner – ASC Transformation role at Liquid Personnel
Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s ASC Transformation team, based in Nottingham. This is an exciting opportunity for an accomplished social work professional to undertake complex reviews and reassessments across a broad range of Adult Social Care services whilst providing leadership, professional guidance, and supporting service transformation initiatives.
Working across Older People, Physical Disabilities, Learning Disabilities, Mental Health, and Supported Living services, you will lead on complex cases, promote strengths-based practice, and support the delivery of high-quality, person-centred social work interventions. Alongside frontline practice, you will play a key role in service improvement, mentoring colleagues, driving innovation, and supporting the implementation of new ways of working across Adult Social Care.
What will your responsibilities be?
- Managing and overseeing complex reviews, reassessments, and casework across a range of Adult Social Care service areas.
- Undertaking Care Act 2014 assessments, reviews, support planning, and complex decision-making to ensure positive outcomes for citizens.
- Reviewing existing care and support arrangements to assess changing needs, outcomes, risks, and opportunities to maximise independence.
- Leading on complex cases involving older people, physical disabilities, learning disabilities, mental health needs, and supported living services.
- Providing professional guidance, mentoring, and practice support to Social Workers and Adult Social Care practitioners.
- Promoting strengths-based, person-centred approaches that support independence, wellbeing, and choice.
- Undertaking safeguarding enquiries, supporting complex safeguarding investigations, and ensuring statutory responsibilities are met.
- Completing Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Working collaboratively with citizens, carers, families, providers, health professionals, and partner agencies to deliver coordinated support.
- Supporting service transformation programmes and contributing to continuous improvement, innovation, and practice development initiatives.
- Assisting with the implementation of new systems, processes, and digital solutions that support effective social work practice.
- Using tools such as Microsoft Copilot, Beam Notes, and electronic case management systems to support efficient recording and reflective practice.
- Contributing to quality assurance activities, audits, and practice reviews to drive high standards across Adult Social Care.
- Maintaining accurate, timely, and legally compliant assessments, reports, reviews, and case records.
Qualifications and Experience:
To be successful in this role you must have;
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care, including complex case management and practice leadership responsibilities.
- Senior Practitioner Experience: Experience supporting colleagues, mentoring practitioners, and promoting high-quality social work practice.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake and oversee complex reviews, reassessments, support planning, and risk management.
- Safeguarding Experience: Experience leading or supporting complex safeguarding enquiries and risk management processes.
- Transformation Experience: Experience contributing to service improvement, transformational change, practice development, or redesign projects.
- Multi-Agency Working: Strong experience working collaboratively with health professionals, providers, carers, families, and partner agencies.
- Digital Skills: Confident using electronic case management systems and digital tools to support high-quality social work practice.
- Communication Skills: Excellent communication, leadership, analytical, report-writing, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice, continuous improvement, and professional development.
