Job Description

RN HOUSE SUPERVISOR

POSITION SUMMARY

The RN House Supervisor provides shift-level clinical and operational leadership for the State Veterans Home. The position functions as the responsible RN during the assigned shift, provides direct and indirect professional nursing care, supervises and supports nursing personnel, coordinates staffing and resident care needs, and acts for nursing leadership within the scope of assigned authority. The role requires independent judgment, timely clinical assessment and intervention, clear communication, and consistent follow-through to support safe, compliant, resident-centered care.

ESSENTIAL PERFORMANCE EXPECTATIONS

The following functions are essential performance expectations of the RN House Supervisor role and directly align with the standardized RN House Supervisor Performance Evaluation:

1. Shift Leadership & Accountability - Assumes responsibility for the home at the beginning of the assigned shift; receives and provides an effective handoff; maintains awareness of resident, staffing, safety, and operational concerns; provides active supervisory leadership; and escalates significant concerns to the DON/ADON/Administrator or other designated leader as appropriate.

2. House Rounding & Staff Oversight - Completes and documents supervisory rounds throughout the home at least every 2 hours and more frequently when resident condition, staffing, safety, or operational needs require. Confirms staff presence and assignment coverage, identifies barriers to care, provides direction, and addresses concerns promptly.

3. Resident Assessment & Clinical Response - Completes or ensures timely RN assessment of significant changes in condition, falls, injuries, acute events, admissions/readmissions, and other events requiring RN evaluation. Initiates appropriate nursing interventions and ensures timely provider, EMS, family/responsible party, and leadership notification as applicable.

4. Documentation & Clinical Follow-Through - Ensures required nursing documentation and follow-up are complete, accurate, and timely, including assessments, progress notes, MARS/TARS, FSBS, antibiotic monitoring, fall/neuro documentation, 7-day documentation, tube/foley monitoring, and other assigned clinical records. Documents RN care and interventions personally performed.

5. Incident/Event Management - Recognizes reportable or significant events; initiates immediate resident safety and clinical interventions; completes required event documentation/RMFs; ensures required notifications and follow-up; and communicates unresolved or high-risk concerns to nursing leadership before the end of the shift.

6. Staffing Management - Maintains accurate attendance and staffing records; identifies call-ins, vacancies, or coverage concerns promptly; completes required staffing grids; uses established staffing procedures to maintain safe coverage; and communicates unresolved staffing concerns to leadership without delay.

ADDITIONAL DUTIES AND PERFORMANCE EXPECTATIONS

  • Medication, Treatment & Direct Care Support – Ensures completion of medications, treatments, wound care, and required clinical tasks. When staffing or resident needs require, assumes direct nursing duties, including medication cart coverage and treatments, within scope of practice.
  • Communication & Shift Handoff - Provides clear, concise, clinically relevant communication to nursing staff, providers, leadership, and the oncoming supervisor. Significant resident, staffing, safety, or operational concerns are communicated before leaving the shift.
  • Time Management & Organization - Prioritizes resident safety, clinical needs, staffing responsibilities, documentation, and administrative duties effectively. Completes required work within the assigned shift without repeated reminders or avoidable carryover.
  • Professional Leadership - Demonstrates professionalism, respectful communication, sound judgment, accountability, teamwork, and visible supervisory presence. Provides direction to staff, supports appropriate delegation, and addresses performance or conduct concerns within assigned authority.
  • Policy, Regulatory & Safety Compliance - Follows ODVA/SVH policy, nursing standards, infection prevention practices, emergency procedures, and applicable regulatory requirements. Identifies and corrects unsafe or noncompliant practices when observed and escalates concerns appropriately.
  • Reliability & Follow-Through - Completes assigned responsibilities consistently, follows through on identified concerns, responds to leadership communications, and demonstrates dependable attendance and readiness to perform the House Supervisor role.
  • Shift-Specific Operational Duties - Completes assigned home/shift processes such as census or attendance reporting, required supply/O2 counts, transport or dietary communication, EMAR backup processes, and other operational duties as established by the home, without replacing the core clinical and supervisory responsibilities of the role.
  • Performs other related duties as assigned consistent with the responsibilities of the position and agency operations.

SUPERVISORY AND CLINICAL AUTHORITY

  • Provides supervision and direction to licensed and unlicensed nursing personnel during the assigned shift and delegates tasks consistent with staff competency, educational preparation, scope of practice, and the Oklahoma Nursing Practice Act.
  • Collaborates with providers and interdisciplinary team members in the management of resident care and acts for nurse managers in their absence within assigned authority.
  • Participates in quality oversight through direct observation, record review, follow-up of clinical concerns, and identification of care or documentation variances requiring correction or escalation.

MINIMUM QUALIFICATIONS

  • Current, unrestricted Oklahoma Registered Nurse license or Registered Nurse license recognized by the Enhanced Nurse Licensure Compact (eNLC).
  • Minimum of two (2) years of professional nursing experience in a field appropriate to the long-term care/health care setting.
  • Demonstrated ability to independently assess resident condition, prioritize care, supervise staff, manage competing shift demands, and communicate effectively with providers, residents/families, and leadership.
  • Long-term care, skilled nursing, charge nurse, or supervisory experience preferred.
  • Candidates must possess sufficient proficiency in the English language to perform the essential functions of the position.
