Registered Nurse (RN) - Start of Care & Clinical Supervisor 

(VA Home Care)

Reports To: Director of Home Care 

Pay Range: $45.00 - $60.00 per hour, based on qualifications and experience.

Location: Colorado Springs, Colorado and surrounding service areas. 

Position Summary 

The Registered Nurse (RN) is responsible for performing comprehensive Start of Care (SOC) assessments and conducting ongoing supervisory visits for Veterans receiving home care services. The RN ensures the delivery of high-quality client care, regulatory compliance, caregiver oversight, and adherence to agency policies, Veterans Affairs requirements, and applicable federal, state, and local regulations. The RN serves as a clinical resource for clients, families, caregivers, physicians, case managers, and agency staff.  

Essential Duties and Responsibilities 

- Conduct comprehensive nursing assessments for new VA clients in their homes. 

- Develop, implement, and update individualized plans of care based on client needs, physician orders, and authorized services.  

- Evaluate physical, psychosocial, environmental, safety, and functional needs.  

- Assess medications, durable medical equipment, risk factors, and overall health status.  

- Coordinate care with physicians, VA representatives, caregivers, case managers, and community resources.  

- Educate clients and families regarding care plans, disease management, home safety, and available resources. [Text Wrapping Break]- Complete all admission and assessment documentation accurately and within required timelines.  

- Identify and report actual or suspected abuse, neglect, exploitation, safety concerns, and significant changes in condition.  

- Conduct scheduled and unscheduled supervisory visits at least every 60 days, or more frequently as required by agency policy, payer requirements, or regulatory standards.  

- Evaluate caregiver competency, performance, professionalism, and compliance with the care plan.  

- Assess client satisfaction and service quality.  

- Review client records and caregiver documentation for accuracy, completeness, and compliance.  

- Provide coaching, education, remediation, and corrective guidance as appropriate.  

- Document findings, recommendations, and follow-up actions after each visit.  

- Report concerns related to quality of care, client safety, compliance, or employee conduct to agency leadership.  

Qualifications  

- Current and unrestricted Registered Nurse (RN) license in the State of Colorado.  

- Minimum one year of nursing experience; home care experience preferred.  

- Valid driver license, reliable transportation, and required automobile insurance.  

- Strong assessment, communication, documentation, and organizational skills.  

- Ability to work independently and collaboratively with interdisciplinary teams.  

- Proficiency with electronic documentation systems and Microsoft Office applications.  

Compliance and Professional Standards  

The RN shall maintain compliance with HIPAA, agency policies, Veterans Affairs requirements, and all applicable federal, state, and local regulations. The employee is expected to maintain licensure, certifications, continuing education requirements, and professional conduct standards as required by law and agency policy.  

Physical Requirements  

Ability to travel to client residences; sit, stand, bend, kneel, and lift up to 25 pounds occasionally; and perform home safety and client assessments in various residential environments.
