Responsibilities
π©Ί What Youβll Be Doing (AKA: Your Superpowers)
- Completing Start of Care (SOC) visits with confidence, compassion, and clinical excellence
- Performing assessments that actually reflect the patientβs needs
- Creating individualized care plans that set the tone for amazing outcomes
- Educating patients and families so they feel empowered, not overwhelmed
- Collaborating with a supportive team that values your judgment and expertise
- Documenting in the EMR (we keep it as smooth and painless as possible)
- Being the RN who makes patients say, βWowβ¦ I feel better alreadyβ
Requirements
β¨ What You Bring
- Active RN license in Michigan
- Strong assessment skills and clinical judgment, including experience conducting OASIS assessments and follow-up visits
- Strong communication skills, including educating patients and families, explaining care plans, and handling Start of Care conversations
- Home health and hospice experience
- Experience collaborating with physicians and therapists
- Reliable transportation and eligibility to work in the United States
- Experience with Kinnser is a plus
