ChristianaCare HomeHealth is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated team caring for primarily geriatric patients throughout the community. Position Highlights
- Full-time, day shift schedule
- One weekend per month
- Two holidays per year
- Limited on-call responsibilities
- Care for patients throughout the community
- Autonomy to manage your daily schedule while supported by an interdisciplinary team
Primary Responsibilities
- Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes
- Perform comprehensive patient assessments and ongoing evaluations
- Develop, implement, and update individualized care plans
- Administer medications, treatments, and IV therapy as ordered
- Educate patients and caregivers regarding disease management, medications, safety, and wellness
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Facilitate discharge planning and transitions of care
- Maintain accurate and timely clinical documentation
Qualifications
- Current RN licensure in Delaware or eligibility through a Compact State License
- BSN preferred; ADN candidates may be considered with a commitment to obtaining a BSN within the designated timeframe
- Minimum of one year of RN experience
- Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred
- IV therapy and wound care experience preferred or willingness to complete training
- Current valid driver's license and reliable transportation

Hourly Pay Range: $36.10 - $57.76This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
