Registered Nurse – Post- Liver Transplant Coordinator

Description:

The Post- Liver Transplant Coordinator serves as the primary clinical liaison between the patient, family, and the multidisciplinary transplant team. This role ensures continuity of care in the outpatient and inpatient settings, monitoring for organ rejection, managing immunosuppression therapy, and maintaining regulatory compliance.

What You Will Do

Clinical Management & Monitoring

• Tracks and reviews post-discharge lab results, diagnostic imaging, and clinical progress to detect early signs of rejection, infection, or medication toxicity.

Medication Management

• Supervises and adjusts immunosuppression and related drug therapies under provider direction, verifying patient adherence and tolerance.

Patient & Caregiver Education

• Instructs patients & caregivers on lifelong medication adherence, infection control, dietary restrictions, and warning signs that require immediate medical attention.

Care Coordination

• Schedules specialty consultations, coordinates outpatient transitions, and manages emergent evaluations or hospital readmissions when complications arise.

Regulatory Compliance & Documentation

• Maintains precise electronic health record (EHR) documentation and reports required clinical outcomes data to bodies like the United Network for Organ Sharing (UNOS) and CMS.

Quality & Research

• Participates in departmental quality improvement projects, clinical protocol development, and transplant registry data tracking.

What You Will Need

• Bachelor's degree in nursing (BSN)

• Current Registered Nurse (RN) license in the State of Illinois
• Current CPR/BLS Certification

• 3 to 5 years of nursing experience in hepatology, gastroenterology, ambulatory care, care coordination, or a related specialty strongly preferred.

Compensation:

Salary Range: $39.48 - 65.14 per hour 

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
