Our Medical Care at Home (MCAH) program provides acute-equivalent care in the community, enabling patients to receive hospital-level treatment at home, work or through virtual care pathways.

Working closely with medical officers, Visiting Medical Officers (VMOs), multidisciplinary teams, carers and community providers, MCAH bridges the gap between hospital and home to deliver safe, evidence-based and compassionate care.

As part of St Vincent's Care at Home, MCAH helps patients avoid unnecessary hospital stays, promotes independence, enhances quality of life and ensures continuity of care through innovative healthcare delivery.

The Opportunity - Registered Nurse, Medical Care at Home

This is a permanent full-time, part-time or casual opportunity based in Toowoomba.

Working as part of the growing Medical Care at Home (MCAH) team, you will provide high-quality nursing care to patients receiving acute and post-acute treatment within the community.

You will work collaboratively with the MCAH Lead (Medical), Visiting Medical Officers (VMOs), St Vincent's Care facilities and a range of healthcare professionals to deliver coordinated, patient-centred care.

As a Registered Nurse, you will assess patient needs, develop tailored care plans, provide clinical interventions and coordinate safe transitions of care.

Your work will directly contribute to reducing hospital admissions, improving patient outcomes and supporting people to recover safely in their preferred environment.

Key Responsibilities:

Clinical Care Delivery

Deliver high-quality, evidence-based nursing care including assessment, care planning, treatment, review and education for patients and carers.

Advanced Nursing Interventions

Provide nursing interventions including IV therapies, complex wound care and monitoring of patients with complex clinical conditions.

Care Planning & Coordination

Assess patient needs and formulate individualised care plans that promote safety, function and independence while coordinating care across services.

Patient & Family Engagement

Communicate effectively with patients, families and carers to support informed decision-making and active participation in care.

Clinical Escalation & Safety

Recognise and promptly escalate out-of-range observations and clinical deterioration in accordance with clinical guidelines and organisational protocols.

Multidisciplinary Collaboration

Work closely with medical officers, allied health professionals and community providers to ensure continuity of care and positive patient outcomes.

Quality & Documentation

Maintain accurate clinical documentation and contribute to quality improvement initiatives, case reviews, safety huddles and service excellence activities.

About You

You are a compassionate and highly skilled Registered Nurse who enjoys working autonomously while collaborating within multidisciplinary teams.

You are passionate about delivering patient-centred care and motivated by the opportunity to help transform healthcare delivery in the community.

You thrive in fast-paced environments, demonstrate sound clinical judgement and are committed to achieving outstanding outcomes for patients receiving care at home.

You Will Bring

• Current registration as a Registered Nurse with AHPRA.
• Minimum 3 years post-graduate nursing experience.
• Experience caring for patients with multiple comorbidities and complex clinical needs.
• Community nursing experience and/or experience across acute medical, surgical or critical care settings.
• Demonstrated clinical knowledge and application of best-practice nursing care.
• Proven ability to work collaboratively within multidisciplinary healthcare teams.
• Commitment to evidence-based, person-centred care.
• Strong digital literacy skills, including the use of clinical systems and virtual healthcare platforms.
• Excellent communication, organisational and problem-solving skills.
• Ability to work independently and manage competing priorities in a time-critical environment.
• Commitment to ongoing learning and professional development.
• Current unrestricted Australian Driver's Licence.
