Position Summary
The RN Field Assessor is responsible for face to face assessing with members to evaluate the medical needs of the member to facilitate the member’s overall wellness and help them obtain the services they need to thrive. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment.
- Minimum of 15-20 assessments per week dependent on need, may be more. Assessments take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Assessments include the member’s level of work capacity and related restrictions/limitations.
Required Qualifications
- Must hold an active unrestricted Registered Nurse license in the state of New Jersey
- Must possess reliable transportation and be willing and able to travel up to 85% of the time. Mileage is reimbursed per our company expense reimbursement policy
- 2+ years clinical practice experience in a hospital, alternative care, home health, ambulatory care setting
- Successful completion of the NJ Choice certification is a contingency of employment. (As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.”)
- Must be able to work Monday - Friday, 8:00am - 5:00pm EST
- Experience using personal computer, keyboard navigation, navigating multiple systems and applications; and using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.
Preferred Qualifications
- Assessment, Case management, Managed Care and/or discharge planning experience
- Crisis intervention experience
- NJ Choice certification (required to obtain with employment)
- Clinical interviewing
Education
- Minimum Associates Degree in Nursing required
- Bachelor's in Science and Nursing preferred

The typical pay range for this role is:
$66,575.00 - $142,576.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
