Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness.  Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.

Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis.

Job Title: Program Manager- Level 1 Care Management (Nashville, TN)

JOB SUMMARY:

The Program Manager of Intensive Services provides leadership, supervision, and support to our Intensive care management services: PACT, CTT, CCFT, and ICC.

 


RESPONSIBILITIES:

Position Responsibilities:   

  • Provide direct supervision to Care Management Supervisors for each Level 1 care management program at Metro Campus. Supervision includes but not limited to in-office meetings, ride-alongs, and chart audits. 
  • Provide leadership, coaching, and support to all Level 1 care management teams across the agency.
  • Monitor and ensure program protocols/policies and procedures are carried out. 
  • Conduct planning sessions and meetings with teams as required by the Agency and as needed. 
  • Promptly address team concerns and work to obtain resolution. 
  • Responsible for on-site consulting with Office Coordinator, Facility Administrator, and Care Managers as needed or assigned. 
  • Provide support and consultation for care managers with consumers in crisis situations. 
  • Ensure consumers are seen upon admission and discharge from the hospital, diversion services, or residential treatment facilities. 
  • Ensure consumer’s daily psychiatric, medical, and environmental needs are planned for and coordinated. 
  • Review care management documentation to ensure this documentation is individualized and meets consumer’s needs and shows medical necessity for continued service. 
  • Ensure consumers are provided with efficient transitions within the Agency and in the community. 
  • Assist teams with referral and linkage to community resources and advocate for consumers when necessary. 
  • Meet regularly with Facility Administrator for supervision.
  • Attend and facilitate team meetings and ensure multi-disciplinary approach.  Ensure appropriate documentation for these meetings. 
  • Participate and facilitate in special staffings for Consumers as needed and/ or requested.

  • Counsel care manager supervisors on performance and complete scheduled performance appraisals.  Ensure care management staff complete scheduled performance appraisals distributed by Human Resources. 
  • Collect and monitor completion of clinical documentation as outlined in Agency policy and procedures, (i.e., hospital, 90-day housing review, etc.). 
  • Collect and review all appropriate documentation for payroll and financial reimbursement and submit to Accounting Department in a timely manner. 
  • Interpret necessary agency reports, use them to monitor outcomes of assigned care managers as outlined in Agency policy and procedure manual. 
  • Assist in the hiring of Care Management staff. 
  • Ensure timely and appropriate documentation as required by the Agency. 
  • Develop and maintain community relations through prompt follow-up with contacts.

Other Responsibilities:

  • Ensure compliance with corporate training policies, safety regulations, and practice standards.
  • Stay informed of industry trends, best practices, and advancements in education.
  • Comply with all Corporate Compliance policies, practices and procedures.
  • Responsible for attending all mandatory in-services, continuing education and annual health requirements.
  • Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director.
  • Comply with all OSHA and safety policies, practices, and procedures.  Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator.
  • Will be compliant with the relevant regulations concerning the privacy and security of consumer’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  • Participate in proactive team efforts to achieve departmental and company goals.
  • Perform other duties as assigned.
  • Provide leadership to others through example and sharing of knowledge/skill.
  • Follow all policies and procedures set by the Agency.

 


REQUIREMENTS:

Essential Skills and Experience:

  • Master’s degree (M.A., M.S.) in a health-related field. 
  • Experience in a mental health setting preferred. 
  • Bachelor’s level in a health-related field with a minimum of 6 months care management team leader experience or 1+ years of care management tier experience. 
  • Excellent oral and written communication skills
  • Excellent organizational, critical thinking, interpersonal, and time management skills
  • Ability to work well independently and in a fast-paced environment
  • Proficient in using relevant technology, including Microsoft Office Suite tools, Google Docs, and database systems
  • Ability to work with diverse personalities with a team-first mindset
  • Ability to build effective relationships with a variety of organizations
  • Excellent leadership, communication, and stakeholder engagement skills.
