As a Primary Care Physician here, you'll manage patients with complex health needs alongside Nurse Practitioners, Care Managers, Pharmacists, Social Workers, and Clinical Support Teams — a team built to share the load so you can spend your time where it matters most: with your patients.

✅ Quarterly Incentive Plan — based on outcomes, quality metrics, and documentation
✅ Long-Term Incentive Eligible — physicians may participate in the Mosaic Health Profit Shares program, rewarding physicians for their contributions to the organization

What you'll need: MD/DO from an accredited medical school, active Arizona medical license, DEA registration, board certified or eligible, and a genuine interest in value-based, patient-centered care.
