Centerville Post Acute is seeking a compassionate, motivated, and licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. Working under the direction and supervision of a licensed Physical Therapist, the PTA plays a key role in helping residents restore mobility, improve strength, reduce pain, and achieve their highest level of functional independence.

Position Summary

The Physical Therapist Assistant provides skilled physical therapy treatments as outlined in the resident's plan of care under the supervision of a licensed Physical Therapist. The PTA works closely with the interdisciplinary team to promote safe, effective rehabilitation while delivering compassionate care that enhances each resident's quality of life.

Essential Responsibilities

  • Administer physical therapy treatments and therapeutic exercises as directed by the supervising Physical Therapist.
  • Assist residents in improving mobility, strength, balance, coordination, endurance, and functional independence.
  • Monitor resident progress and communicate changes to the supervising Physical Therapist.
  • Accurately document treatments, resident responses, and progress in the electronic medical record.
  • Educate residents and family members on exercise programs, safety precautions, and proper use of assistive devices.
  • Assist with transfers, gait training, therapeutic activities, and functional mobility exercises.
  • Participate in interdisciplinary care planning meetings.
  • Maintain treatment areas, equipment, and supplies in a clean and safe condition.
  • Follow all facility policies, state and federal regulations, and Medicare documentation requirements.
  • Collaborate with nursing staff, occupational therapists, speech therapists, physicians, and other members of the healthcare team.

Qualifications

  • Graduate of an accredited Physical Therapist Assistant program.
  • Current Physical Therapist Assistant license in the State of Ohio.
  • CPR certification required.
  • Minimum of 2 years of Physical Therapist Assistant experience preferred, with skilled nursing or post-acute care experience strongly preferred.
  • Excellent communication and interpersonal skills.
  • Experience with electronic medical records preferred.

Physical Requirements

  • Ability to stand, walk, bend, kneel, and assist residents throughout the workday.
  • Ability to safely lift, transfer, reposition, and assist residents using appropriate equipment and safe patient handling techniques.
  • Ability to occasionally lift up to 50 pounds.
