Job Description:

Position Summary

Supports safe, accurate, and effective medication-use processes across inpatient, outpatient, procedural, and ambulatory care settings through clinical pharmacy services, medication order verification, dispensing oversight, compounding activities, medication safety initiatives, regulatory compliance, and interdisciplinary collaboration.

Required Qualifications

· PharmD or Bachelor of Science in Pharmacy from an ACPE-accredited school of pharmacy.

· Current pharmacist license in good standing with the Texas State Board of Pharmacy.

· Ability to maintain pharmacist licensure and required continuing education.

· Successful completion of an ACPE-accredited sterile compounding (IV) certification program.

· Ability to maintain sterile compounding competency in accordance with organizational policy, USP standards, and regulatory requirements.

Knowledge, Skills, and Abilities

· Knowledge of medication-use processes, medication therapy assessment, medication safety principles, and pharmacy operations.

· Knowledge of federal and state pharmacy regulations, DEA requirements, Texas State Board of Pharmacy regulations, Joint Commission medication management standards, USP standards, and applicable organizational policies.

· Ability to perform medication order verification, clinical evaluation, dispensing, and compounding functions.

· Ability to identify medication-related problems and provide clinical recommendations.

· Ability to direct pharmacy technician workflow.

· Ability to support regulatory compliance, accreditation readiness, inspections, audits, and quality improvement initiatives.

· Effective communication, interpersonal, and computer skills.

· Ability to manage competing priorities while maintaining accuracy, safety, and operational efficiency.

Preferred Qualifications

· Hospital pharmacy experience.

· Pediatric pharmacy experience.

· Experience with Epic Willow (Inpatient and Ambulatory) and/or Omnicell.

· Experience with medication safety, medication stewardship, quality improvement, regulatory compliance, or accreditation readiness activities.

Essential Duties

· Performs medication order verification and clinical review of medication therapy.

· Evaluates and optimizes medication therapy to improve safety and patient outcomes.

· Verifies and authorizes medication dispensing activities.

· Maintains medication profiles and supports medication reconciliation and patient education activities.

· Performs or oversees sterile and nonsterile compounding activities.

· Directs pharmacy technician workflow.

· Supports medication safety, quality improvement, formulary management, and medication stewardship initiatives.

· Maintains compliance with federal and state pharmacy regulations, DEA requirements, TSBP regulations, Joint Commission standards, USP standards, and organizational policies.

· Participates in DEA, TSBP, Joint Commission, and other regulatory or accreditation inspections, surveys, audits, and readiness activities.

· Collaborates with healthcare professionals to support safe and coordinated patient care.

· Maintains readiness for changing workload demands and urgent clinical situations.
