Oak River Rehab, a skilled nursing facility in Anderson, California, is seeking a dedicated and experienced MDS Coordinator to join our clinical leadership team.

Position: MDS Coordinator
Location: Anderson, CA
Pay: Starting at $45.00/hour and up, depending on experience

Requirements:

  • Current LVN or RN license in good standing
  • Experience with MDS assessments and care planning preferred
  • Knowledge of Medicare, Medicaid, and reimbursement regulations
  • Strong organizational and communication skills
  • Ability to work collaboratively with interdisciplinary teams

Responsibilities:

  • Coordinate and complete MDS assessments accurately and timely
  • Ensure compliance with federal and state regulations
  • Develop and update resident care plans
  • Collaborate with nursing, therapy, dietary, and social services departments
  • Monitor documentation and reimbursement processes
