Overview:

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Social Worker - MSW!

This position is eligible for up to a $15,000 sign-on bonus for qualified candidates.

Status: Full-Time, permanent position (40 hours)

Standard working hours: 8:00 AM – 4:30 PM EST, Monday–Friday, with rotating weekends (Saturday or Sunday). A weekday off is provided when working a weekend day so hours do not exceed 40 per week.

Location: This is an onsite position at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507.

Job responsibilities:

• Responsible for the provision of medical social work services to patients receiving care in the hospital setting.

• Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient.

• Collaborates with healthcare team members for the patient and community organizations to identify needs and resources in the hospital and community.

• In consultation with the healthcare team develops effective transition plans.

• Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay.

• Functions in one of the following practice settings: Hospitals Only

Education:

• MLD - Master's Level Degree in Social Work / MSW REQUIRED

Certification/Licensure 

• Basic Life Support (BLS) - Certification within 90 days of hire.

• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

Experience:

1 year of acute care and/or healthcare experience requirement is defined below:

• Educational internship experience in an acute care facility can be considered OR

• Healthcare experience refers to the care delivered in one of the following settings: long-term care, home health, hospice, Skilled Nursing Facility (SNF) ​

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $72,737.60 - $110,323.20 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
