This role involves interstate travel for program delivery and stakeholder engagement and requires someone who thrives in a dynamic environment, manages competing priorities effectively and remains focused on delivering exceptional client outcomes.

This role is offered as a Fixed Term Contract until June 2028 and can be based at either of our  Hobart, TAS or Chermside, QLD offices.

  • Deliver services via phone, Telehealth, face-to-face, or email in line with DSA's psychosocial case management principles.
  • Escalate uncertain client cases to the line manager for review.
  • Maintain accurate and up-to-date client records and data in the CRM system, ensuring confidentiality and data security.

What do we need from you?

  • Experience in Australian health, hospitals or aged care (community or residential).
  • Knowledge of the types of dementia, their presentation, and strategies to manage behaviour changes.
  • Excellent problem solving, analytical and decision-making skills.
  • Be relatable and able to share knowledge in a way that supports change at all levels.

  • Great written communication and report-writing skills.
