About the role
- The Community Re-entry Support Service (CRESS) Aboriginal Health Practitioner provides culturally safe, holistic, and person-centred healthcare to Aboriginal people experiencing homelessness in inner city Sydney with complex unmet health needs and frequent justice system contact.
- Work alongside clients, the CRESS Coordinator, multidisciplinary teams and communities to deliver specialist healthcare, consultancy, and shared-care coordination to improve access to healthcare, accommodation, and support.
- Provide care within an assertive outreach model, undertaking clinical responsibilities within scope of practice, including assessment, screening, intervention, health education, and interpretation of relevant clinical assessments and procedures
- Apply trauma-informed, harm reduction, and strengths-based approaches to support Aboriginal clients through advocacy, care planning, and connection with appropriate health, accommodation, and community services
- Develop and maintain partnerships with Aboriginal communities, service providers, and key stakeholders, including justice, housing, and health services, to strengthen care pathways, address service gaps, and support improved service delivery
- Contribute to service development, quality improvement, research, and evaluation activities while maintaining professional accountability through effective communication, documentation, data reporting, and adherence to relevant policies and governance requirements
About you
- Must be of Aboriginal and/or Torres Strait Islander descent and have demonstrated ability to effectively and sensitively liaise and communicate with local Aboriginal and Torres Strait Islander people, communities and agencies
- Hold or be willing to work towards a Certificate IV in Aboriginal Primary Health Care (Practice), and eligibility to be registered with the Australian Health Practitioner Regulation Agency (AHPRA)
- Experience working with people experiencing homelessness and contact with the justice system, with an understanding of the barriers impacting access to healthcare, support and accommodation
- Strong verbal and written communication skills, with the ability to build effective relationships and collaborate with stakeholders across healthcare, community, and custodial settings
- Ability to work autonomously, demonstrate initiative, and contribute to community development, quality improvement, evaluation, and research activities, with a commitment to addressing health and justice issues impacting Aboriginal communities
- Ability to work collaboratively within a multidisciplinary team to provide integrated healthcare and support to Aboriginal people experiencing complex health and social needs
