Compensation: $78,000 to $90,000 / year plus commission and up to 8% yearly bonus

How you will make an impact

• Perform onsite clinical collection of data of the referrals to determine appropriateness for transfer to Continuing Care.
• Perform discharge planning and care coordination for residents transferring from acute hospital to Continuing Care or returning to Independent Living at the Erickson Living community.
• Collaborate with physicians regarding anticipated discharges from facilities.
• Identify quality issues and participates in quality management activities, as needed, with Administrator and/or Medical Director.

What you will need 

• CCRC, Skilled Nursing, Memory Care / Assisted Living experience highly preferred.
• Liaison experience or healthcare marketing background preferred.
• Familiarity with acute hospitals, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred.

As the Clinical Liaison, you are responsible for the data collection and coordination of admissions for referrals from inpatient acute care and sub-acute care settings, including internal residents of the Erickson Senior Living community. You will collaborate with Physicians, Continuing Care Sales/Admissions Staff, Hospital Case Managers, Social Workers, and/or Discharge Planners to facilitate the admissions/transfer process.
