The Certified Diabetes Educator II plans and provides individual and group instruction to meet requirements established in the National Standards for Diabetes Self-Management Education Programs and American Diabetes Association for participants in the Diabetes Self-Management Program. provides advanced and specialized knowledge and communicates recommendations for changes in patient treatment to physicians and hospital staff; documents recommendations and monitors changes and outcomes; facilitates the development and implementation policies, procedures and protocols related to diabetes management. Serves as resource and mentor to non-certified diabetes educators.The Certified Diabetes Educator II
Applies advanced clinical knowledge to assess patients, provide education, and communicate treatment recommendations to physicians and care teams, monitoring outcomes and documenting interventions.
Develops and implements diabetes management policies, procedures, and protocols and supports performance improvement initiatives through data tracking and outcome evaluation.
Collaborates with educators and program leadership to develop, revise, and deliver educational materials and curriculum aligned with ADA standards.
Conducts age-specific assessments and develops individualized education plans based on patient needs and clinical data.
Reviews and interprets blood glucose reports and clinical data to identify patient needs and establish appropriate plans of care.
Coordinates and delivers group and individual education sessions for patients, staff, and community audiences.
Provides follow-up and case management support, including coordination with case management services as appropriate.
Supports development and execution of quality improvement and program evaluation initiatives.
Participates in multidisciplinary team activities to support comprehensive diabetes care.
Serves as a resource and mentor to non-certified diabetes educators and supports staff education and training.
Assists with coordination of program resources and activities in collaboration with program leadership.
May serve as program coordinator, overseeing planning, implementation, evaluation, and reporting of diabetes education services, including ADA credentialing requirements.
Ensures compliance with program standards, including educator qualifications, documentation, and reporting requirements.
What We Require:
Education: Graduate of an accredited school of nursing or dietetics program required.
Experience: Minimum 1 year clinical experience working with a diabetic population required.
Licensure, Certification, and/or Registration: Current RN licensure in the State of Virginia or Registered Dietitian (RD) credential required. Certified Diabetes Care and Education Specialist (CDCES) required or must obtain within 12 months of hire.
Other Minimum Qualifications: Excellent oral and written communication and interpersonal skills required. Ability to work independently and demonstrate strong organizational skills. Ability to effectively teach individuals from diverse socioeconomic and cultural backgrounds. Competency in curriculum development and instruction. Computer skills preferred.

Compensation Range:
$33.52 - $50.28Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
